Description

The PCO-EAST (Program Contracting Office) East intends to negotiate on a sole source basis with Healthy African American Families Phase II, 4305 Degnan Blvd. Ste 105, Los Angeles, CA 90008-4949 to obtain services that will support an approved VA Research project titled "Improving Patient-Centered Care Coordination for High-Risk Veterans in PACT (PACT QUERI). The scope of work being requested will be to provide the necessary personnel and supplies to convene quarterly community stakeholder groups while gathering data that will aide in ongoing studies regarding the improvement of care coordination for Veterans. This notice of intent is not a request for competitive proposals. Any responsible sources who wish to identify their interest and capability to provide this equipment or equipment that is compatible must notify this Contracting Officer at Bridget.Kornish@va.gov NLT Friday April 14, 2016 by Noon EST. Telephone responses will not be accepted. Upon receipt of the interested parties' notification, the Contracting Officer will determine whether to conduct a competitive procurement or proceed with sole source negotiations.
