Quality Assurance Surveillance Plan - Attachment 4 Invoicing Instructions - Attachment 3 Disclosure of Lobbying Activities - Attachment 2 Statement of Work - Attachment 1 RFP THIS IS A NOTICE OF INTENT AND NOT A REQUEST FOR QUOTATION. The National Institutes of Environmental Health Sciences (NIEHS), Research Triangle Park, NC intends to negotiate on a sole source basis with Carl Zeiss Microscopy LLC of Thornwood NY for the purchase of "Preventive Maintenance and Repair of Government Owned Carl Zeiss Microscopy Instruments", in support of the Fluorescence Microscopy and Imaging Center (FMIC) involving researchers assess to a variety of techniques that push the envelope on 3D cellular imaging. This acquisition is conducted under the authority of the Federal Acquisition Regulations (FAR) Subpart 13, Simplified Acquisition Procedures (section 4202 of the Clinger-Cohen Act of 1996) in conjunction with FAR Part 12, Acquisition of Commercial Items. The FAR as supplemented by the Health and Human Services Acquisition Regulation (HHSAR) will be used to evaluate the offer and award this requirement. The North American Industry Classification System (NAICS) code for this requirement is 811219, "Other Electronic and Precision Equipment Repair and Maintenance" with a size standard of $20.5M. The Contractor shall provide the NIEHS/FMIC with qualified personnel to perform a comprehensive preventative maintenance plan including inspection, preventive maintenance, repair service, and software updates/maintenance as required by the FMIC at the National Institute of Environmental Health Sciences (NIEHS), Research Triangle Park, North Carolina. The contractor shall furnish all necessary services, parts, labor, software upgrades, travel, and qualified technical personnel who are factory trained to work on Zeiss Confocal microscopes. Complete mechanical, electrical, and software preventative maintenance response, including necessary repair service, shall be provided by the contractor for Government-owned confocal microscopes. The contractor shall be held responsible for the cost of any repairs caused by negligence and abuse on their part. The contractor shall be responsible for safeguarding all Government property that will be serviced under this service agreement. The contractor shall provide factory trained personnel with experience in the repair and maintenance for Zeiss LSM 710, LSM 780, and LSM 880 equipment types. Personnel must be qualified to perform the required preventive maintenance services and repairs for all listed equipment. All workmanship performed by the contractor shall be of acceptable industry standard quality and shall be immediately rectified at no additional cost to the Government upon notification of non-acceptability by the Government. The contractor shall provide a minimum of one (1) preventive maintenance visit per year and repair service (as needed) for all listed Government-owned confocal microscopes consistent with standards of workmanship materials prevailing in the trade. Preventive Maintenance (PM): Scheduled checks and services (including, but not limited to, adjustments, cleaning, calibration, inspection and equipment servicing) of installed equipment at intervals recommended by the manufacturer or standards within the industry to ensure proper and efficient operation with minimum breakdowns, deterioration, or other deficiencies. Repair Service: The restoration of failed or failing Government-owned property to its intended capability or use. Since minimizing downtime is critical for this equipment this contract must include preferred service call scheduling where the government is placed at the top of the repair engineers scheduling (this priority service will place NIEHS ahead of third party and time and material customers). Repairs may be accomplished by replacement or restoration of component parts or materials that have deteriorated, worn out, or are broken and unable to perform to current prescribed standards. Additionally, the contractor shall provide unlimited collaborative service visits, technical support, and applications support as well as software updates for these laser scanning confocal microscope systems. This notice of intent is NOT a request for competitive proposals; however, all responses received within 10 days from the date of publication of this synopsis will be considered by the Government. A determination by the Government not to compete this proposed contract action based upon responses to this notice is solely with the discretion of the Government. Information received will normally be considered for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 10 days of this notice to determine whether a qualified source is more advantageous to the Government, the order will be issued to Expression Analysis. There is no solicitation package available. The Contracting Officer for this requirement is Ms. Onyenauché Washington.
