- THIS RFI IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH FAR 15.201(e). PRICING SHALL ONLY BE USED FOR PLANNING PURPOSES AND SHALL NOT BE USED AS AN EVALUATION AGAINST ANY POTENTIAL OFFEROR SHOULD A SOLICITATION ARISE FROM THIS RFI. DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification, hubzone, 8(a), small, small disadvantage, veteran owned small business or service disabled veteran owned small business) relative to NAICS 541620. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office is seeking a contractor to provide VA Healthcare System (VAHCS) San Diego all necessary labor and materials to conduct review of the facility Storm Water Pollutant Prevention Plan (see Statement of Work below). The information identified above is intended to be descriptive, not restrictive and to indicate the quality and services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the services that fulfill the required specifications. If a solicitation is published, a site walk thru may be available in order to address key details. If you are interested, and are capable of providing the sought out services please provide the requested information as well as the information indicated below. Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: (1) Is your business small; (2) If small, does your company qualify as a small, emerging business, or small disadvantaged business; (3) If disadvantaged, specify under which disadvantaged group and if your company is certified under Section 8(a) of the Small Business Act; (4) Is your firm a certified "Hubzone" firm; (5) Is your company a woman-owned or operated business; (6) Is your company a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business; (7) If your company has FSS GSA contract, please provide the contract number and (8) your capabilities in regards to the items being researched. Responses to this notice shall be submitted via e-mail only at Claudia.lankford@va.gov Telephone responses shall not be accepted. Responses must be received no later than FRIDAY by 4:00 p.m. PST on April 15, 2016. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this sources sought. Responses to this sources sought notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Statement of Work (attached below) STATEMENT OF WORK (SOW) Storm Water Pollution Procurement Package 1. Background. The VASDHS at 3350 La Jolla Village Drive, San Diego CA, CA 92161 is part of the Desert Pacific Healthcare Network (VISN 22), and is one of the most diversified tertiary health care providers in the VA. It is a teaching hospital, providing a full range of patient services, with state-of-the art technology as well as education and research. 2. Scope. Contractor shall provide all necessary labor and materials to conduct review of the facility Storm Water Pollutant Prevention Plan -SWPPP- and implement A SWPPP as mandated by California order No 2013-0001-DWQ and general permit No CAS000004. Contractor shall develop and implement the required five-year plan as mandated in the general permit requirement to ensure compliance of the VASDHS storm water program with current regulations. Contractor shall conduct site survey and field sampling of storm water across the facility, conduct maintenance of the storm drain system, developing and implementing a pollution prevention plan including inspection, system monitoring, and implementation of corrective actions and pollution prevention training. Contractor shall enact and apply necessary measures to correct all noted deficiencies. Contractor shall be licensed, and shall be a Qualified SWPPP Developer. Contractor shall take necessary steps to submit Permit Registration Documents (PRDs) on behalf of the VASDHS. 3. Specific Tasks. Contractor shall conduct the following tasks: 1.Submit a notice to Intent to the State Water Resource Control Board: Contract shall submit VASDHS Permit Registration Documents (PRDs) to the State Storm water Resources Control Board -SWRCB. 2.Review VASDHS legal Authority to implement program within the VASDHS 3.Education and outreach 4.Public involvement 5.Illicit discharge and elimination 6.Construction site storm water control 7.Pollution prevention and good housekeeping 8.Water quality monitoring 9.Program effectiveness evaluation and improvement program 10.Conduct survey and inspection of the storm water system across the facility to detect/ identify existing deficiencies. This includes but is not limited to collection and discharge points at buildings roofs, storm water auxiliary equipment (piping & other valves, grates ¦). Contractor shall provide accredited certified personnel to conduct required work. 11.Handling of illicit discharges shall conformed to requirements as described in order No 2013-0001-DWQ. 12.Contractor activities across the facility shall be coordinated with the COR and VASD Engineering service; these activities shall create minimal to no disruption to the facility routine operation. 4. Deliverables: Report and others deliverables shall conform to the timeline described in order No 2013-0001-DWQ Contractor shall submit the annual summary of the mandated tasks outlined in the SWPPP. Contractor shall certify activity compliance with the SWPPP as outlined in order No 2013-0001-DWQ and general permit No CAS000004. Report shall be submitted to local authorities and to VASDHS project's COR. Submit one (1) hard copy and one (1) electronic copy of the complete written report at reporting periods mandated in the SWPPP in accordance with order No 2013-0001-DWQ and general permit No CAS000004. All reference materials shall be included in the electronic version of the report in MS Word and PDF, either through scanning or other means of electronic text import methods. Information shown in the reports shall be as follows: 1.Company's name, address, telephone & fax numbers. 2.Name and signature of contractors who perform the sample collection and testing. 3.VAMCSD Work Contract Number, name and number of CO. 4.Date and Time of work. 5.Sampling location, Building Name, and information of samples collected and tested. 6.Test data, location, and results 7.Recommended corrective actions, if any. 8.Cost estimates for sampling and testing. A rough order of magnitude cost estimate for any recommended repairs or removals. 9.List of all suspect materials in the system with assessment of their condition and quantifying of the level of contamination and corrective actions 10.Drawings/Pictures showing the sampling locations. 11.Storm water Pollution Prevention Plan to include to include, procedures for remediation of existing hazardous materials with positive sample, training plan and implementation of 5. Performance Monitoring Contractor performance shall be monitored by the VASD COR. The COR can perform inspection of the work performed without the presence of the contractor. COR reserves the right to enroll third party expertise service to evaluate and validate work performed by the contractor. 6. Security and Safety Requirements a.All Contractor personnel are required to wear I.D. badges during the entire time he/she is on Government property. I.D. badges must have an identification picture and shall state the name of the individual and the company he/she represents. b.It is the responsibility of the contractor's personnel to park in the appropriate designated parking areas. Parking information is available from the participating healthcare center's Security Office. VA San Diego Healthcare System will not validate or make reimbursement for parking violations of the contractor's personnel under any circumstance. c.For repairs or services required outside normal working hours, the Contractor is required to check-in with the Security Officer on duty and with VA San Diego Bldg. 1 prior to start of repairs or service. Check-in and check-out procedures defined above are mandatory and will be strictly enforced. d.The Contractor shall comply with all applicable Federal, State, and local requirements regarding workers' health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926. The Contractor is solely responsible for determining the legal requirements that apply to activities, and shall ensure safe and healthful working conditions for its employees. e.In the performance of this contract, Contractor shall take such safety precautions as necessary to protect the lives and health of the occupants of the building. Fire and safety deficiencies, which exist and are part of the responsibility of the Contractor, shall be immediately corrected. The Contractor shall comply with applicable Federal, State, and local safety and fire regulations and codes, which are in effect during the performance period of the contract. f.All necessary precautions shall be taken by the Contractor to prevent accidental operation of any existing smoke detectors or sprinkler system. 7. Other Pertinent Information or Special Considerations. Hospital operation shall be maintained during the entire duration of the service. a.Identification of Non-Disclosure Requirements. Sharing of digital drawings files such as AutoCAD drawings required signature of a Digital Data Licensing Agreement release form between VASDHS and outside entities. Contractor shall notify COR if such need present itself. b.Packaging, Packing and Shipping Instructions. Contractor shall not store within the facility, materials required for the work, past the duration of a scheduled day worth of work. c.Inspection and Acceptance Criteria: Contractor performance shall be evaluated using but not limited to the following criteria: 1.Ability to perform expected work and provide reports at the specified timeframe. 2.Quality of work performed and quality of 3.Familiarity with VA procedures and specifically, the VASDHS facility. 4.Contractor's staff qualification, experience, and professional approach toward VASDHS employees and Veterans. 8.Place of Performance: 3350 La Jolla Village Drive, San Diego CA 92161 9.Period of Performance: One Base Year Contract and 4 Option Years. Contractor is not required to provide service on the following Federal holidays, nor will the Contractor be paid special rate for these holidays.
