- 1)Change paragraph 4 of the pre-solicitation notice as reads "Part I of the SF 330 should not exceed 55 pages (single sided), including 25 pages for Section H. Font size shall be 10 point Times New Roman Font or greater" to read "Part I of the SF 330 should not exceed 65 pages (single sided), including 25 pages for Section H. Font size shall be 10 point Times New Roman Font or greater." Other conditions remain unchanged. 2)All pages shall be 8 ½" x 11". 3)Small Business Subcontracting Plan pages are excluded from the page limit. 4)Change Factor 1-Technical Capabilities, I). Firm's Professional Qualifications as reads "The firm must show the firm's longevity as a firm in good standing, the firm's proposed team successes working together for the firm and the firm's involvement working on those projects. Firms shall provide projects that are relevant and recent to the Task Groups along with client contact information" to read "(i) The firm must show the firm's longevity as a firm in good standing, the firm's proposed team successes working together for the firm and the firm's involvement working on those projects. Other conditions remain unchanged. 7)Appendix I add the following qualifications: a)Medical Equipment Planner(s) - Shall have a minimum of 5 years' experience as a Medical Equipment Planner, bachelor's degree (minimum) in biomedical engineering, healthcare administration or an allied health discipline; have a working knowledge of US building codes, scheduling, and state of the art methods associated with medical facility construction. Experience in developing equipment lists, interpreting medical equipment needs from clients, performing and analyzing inventory of client's existing medical equipment, as well as participating and leading client meetings. b)Space Planner(s) - Bachelor's degree in architecture, healthcare or business administration, or related fields AND five years of facilities space management experience. Knowledge of space utilization and facilities management, workload analysis, budgetary practices and principles. Demonstrated ability to develop, establish and control methods, procedures and systems to optimize effectiveness of facilities utilization and management, long range planning for facilities projects, scheduling work projects, monitoring work in progress, and inspection of finished projects. 8)Appendix II CFM Region Map shall include VISN 9, which includes Kentucky and Tennessee. 9)Change "Note 1" as reads "Additionally, submit a signed JV agreement. Failure to submit a signed copy of the JV agreement will result in the JV's submission being excluded from consideration for award" to read "A signed letter of intent to form a JV can be included with the SF 330. However if chosen to negotiate there must be a signed JV agreement in place before negotiation. Failure to submit a signed copy of the JV agreement will result in the JV's submission being excluded from consideration for award". Other conditions remain unchanged.
