Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures, specifically FAR Subpart 13.5 - Simplified Procedures for Certain Commercial Items; and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation is attached to this announcement. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation # VA262-16-Q-0574 is issued as a Request for Proposal (RFP) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award with a base year and up to four (4) option years. This procurement will be issued under as an unrestricted competitive requirement. The North American Industry Classification System (NAICS) Code is 621399 and the Small Business Administration's (SBA) size standard is $7.5 million. The Government is seeking a contract for two (2) board certified Medical Physicists, one (1) Doctor of Philosophy (Ph. D) and one (1) Master of Science (M.S.) Medical Physicist, to provide onsite radiation oncology services at the VA Greater Los Angeles Healthcare System (VAGLAHS). Onsite radiation oncology services shall meet or exceed any current or future requirements approvals or certifications as may be levied by applicable legal, regulatory, accrediting body, governing body, and/or local/state/federal requirements as required by law (e.g. American College of Radiology Guidelines for Radiation Oncology, Veterans Affairs' standard operating procedures, etc.). See attached SF 1449 Solicitation for Performance Work Statement (PWS), applicable clauses / provisions, and other pertinent information relative to this solicitation. Place of Performance: VA Greater Los Angeles Medical Center (VAGLAHS), 11301 Wilshire Blvd, Los Angeles, CA 90073. Period of Performance: Base year and up to four (1) one (1) year option periods. Provisions 52.212-1, Instructions to Offerors - Commercial, 52.212-2, Evaluation - Commercial Items, 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Tradeoff evaluation process as such, an award will be made to the offeror who conforms to the solicitation and represents best value to the Government. Submitting Offer: Offeror(s) shall submit their offer on a company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items or a copy of offeror's annual offeror representations and certifications (ORCA) document from www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items. As an alternative, Offeror(s) may use the Price/Cost Schedule of the attached SF 1449 for offer submission.
