- Hazardous Energy Control Form Statement of Work - Attachment 1 This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Part 13, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on FEDBIZOPPS (www.fbo.gov). The RFQ number is N39040-16-T-0159. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-87 and DFARS Change Notice 25 March 2016. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at the following address: http://farsite.hill.af.mil/. The NAICS code is 811310. The small business size standard for NAICS Code 811310 is $7.5 million. This is a Sole Source (restricted) solicitation action to Machine Tool Research, Inc. due to the proprietary technical knowledge needed and the use of OEM (Original Equipment Manufacturer) parts of Betts-Bridgeport lathes. Portsmouth Naval Shipyard requests responses from qualified sources capable of providing: Repair of the headstock, adjustment of the tailstock, and the realignment of the bed sections of Betts-Bridgeport lathe S/N E-5220 in accordance with Statement of Work listed as Attachment 1. Period of Performance: 1 May 2016 through 30 July 2016. Place of Performance is Portsmouth Naval Shipyard, Kittery, ME.
