Description

COMBINED SYNOPSIS/SOLICITATION: The Department of Veterans Affairs on behalf of Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042, intends to issue a Firm-Fixed price Service Contract Award to a Service Disabled Veteran Owned Small Business (SDVOSB) to provide Veteran Wellness services. The North American Industry Classification System Code (NAICS) is 713940 and the small business size standard is $7.5 million. The Solicitation issue date will be approximately April 28, 2016, with an approximate response date of April 25, 2016. Contractors are encouraged to read the entire Solicitation and send questions to the Contracting Officer by April 22, 2016. The Solicitation will be a Request for Quote (RFQ) and will be issued as a 100% Small Business set-aside. This requirement is designed to provide a wide variety of health and wellness services to support and supplement programs currently in place at the Lebanon VAMC. The basic requirements are: Staffing "3 staff will be on duty at all times during normal business hours - 8:00A-4:30P Monday -Friday. o1 Supervisor or Point of Contact o1 staff Certified in Lifeguard/Water Safety when the pool is operational o1 additional staff person Orientation "Contractor shall provide orientation to the gymnasium facilities as needed. "Contractor shall provide orientation to the gymnasium facilities for follow-up appointments. "Contractor shall be available for questions/program assistance for Veterans utilizing the facilities. Fitness Classes "Contractor shall host fitness classes. Possible classes include, but are not limited to: oAquatic Fitness oBootcamp oAerobic/Step oYoga oZumba Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of mobile Lithotripsy services. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 713940 and the small business size standard is $7.5 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.Provide a summary of the type of preventative maintenance performed on the Lithotripsy equipment. 4.Provide the name of the Fitness and Wellness services used and a summary of the advantages to patient care. 5.Provide a description of the type of table used and a summary of the advantages. 6.Provide the minimum turnaround time in which service can be restored should equipment malfunction or breakdown. 7.Provide the minimum time of availability to provide services upon being given notice. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
