- LIMITED SOURCE JUSTIFICATION Defense Logistics Agency (DLA) Troop Support, Medical Supply Chain, Electronic Catalog (ECAT) Division, is the contracting activity. The justification is for the acquisition of Laerdal Medical Simulation: 214-02001. This order will be placed against Beacon Point Associates' ECAT contract SPE2DE-15-D-0012, which expires February 11, 2020, or other competing ECAT vendors. The Medical Education & Training Campus (METC), Fort Sam Houston has the following requirement: Item: Laerdal Medical Simulation 214-02001 BPA Part Number: 214-02001 National Stock Number (NSN): NA Quantity: 15 Each Unit Price: $45,376.77 Estimated Dollar Value: $680,651.55 This action is being conducted under the Multiple Award Schedule Program. The authority waiving the competition requirement is FAR 8.405-6 (b)(1) - only one manufacturing source is capable of providing the supplies or services required at the level of quality training required. The supplies or services ordered are highly specialized and unique to Laerdal Medical, come with LEAP software that makes it possible to use the METC current inventory of SimMan Essentials simultaneously and, allow the facility to continue the lifespan of current inventory instead of replacement. This requirement meets the needs the best and is supported by AFMMAST (Air Force Medical Modeling and Simulation Training) on curriculum, user training and scenario development. In the interest of economy and efficiency, the new supplies are a logical follow-on to an original delivery order. The Laerdal Medical simulation and manikin is the only compatible model that will meet the clinical and technical requirements of the METC, Fort Sam Houston. The simulators are required for the certified Basic Medical Technician Corpsman Program (BMTCP) and are compatible with other simulators currently used at METC. The SimMan Essential technology was developed with unique application software within every Laerdal patient simulation system. The LEAP application, based on patented human physiological and pharmacological models, permits life-like simulator reactions to treatment and interventions. These patented models are licensed exclusively to Laerdal. The LEAP software also bridges the gap of user interface training. And, it allows for seamless transition between each simulator due to the consistency of the LEAP software. •- Minimum clinical performance requirements: scenarios for pharyngeal obstruction, laryngospasm, decreased lung compliance, airway increased lung resistance, stomach decompression, airway complications, realistic chest rise and fall, inflatable tongue edema, circulation, custom scenario programming, simulations in Nursing Education and Nursing Education Vol II, Advanced Cardiac Life Support scenarios, rapid response, emergency preparedness, airway occlusion, foreign object obstruction, chest tube insertion, intubation, oral/nasal airway insertion, endotracheal tube insertion, oropharyngeal and nasopharyngeal airways insertion and suctioning, suctioning techniques, detailed CPR evaluation, abdominal thrust and other specific medical procedures. - Input: 100-240 50/60 Hz, Consumption: 400 W. - Operator and biomedical equipment repair training. Market research was conducted June 2015 using the internet to research and evaluated manikins and simulators currently available in the marketplace. In collaboration with AFMMAST, METCs' Education and Training section researched the best simulators of three vendors and reviewed all manikins and human patient simulators offered. Laerdal Medical, Gaumard and CAE products were considered and evaluated. Five Laerdal Medical simulators were evaluated. The Laerdal brand was the only product that had interoperability with other currently used Laerdal products in the METC inventory and the LEAP software. In addition, it is the only product in the industry able to give the physiological response needed for the mission. Upgrading the entire METC simulation center to all Laerdal Medical simulator products would prove cost prohibitive. Evaluations of the best vendor manikins and simulators were compared with the AFMMAST requirements metric currently used by the Air Force to evaluate medical training objectives. (See attached). Using the metric, the SimMan Essential manikin met 42 of the requirements for the certified METC training course. Another consideration given to Laerdal products was product compatibility with METC inventoried Laerdal products which have proven durability according to operator training instructors and biomedical technicians within METC. Five Gaumard simulators were evaluated. Gaumards' best product for the simulation course training, the Gaumard Trauma HAL, met only 34 of the METC/AFMMAST metric requirements. Cost to replace all simulators with these products was also considered cost prohibitive and the manikins were not interoperable with current inventories. Five CAE models were evaluated. Two CAE simulator models, the CAE MMP and CAE HPS Adult, met 35 of the training requirements. These systems were not compatible with the METC inventory and did not offer software compatibility. Assessments of all candidate manikin/simulators by METC using the AFMMAST requirements metric for training objectives were conducted to make the final product item determinations. DLA Medical ECAT Division has already determined the price of the Laerdal Medical products on two ECAT vendor contracts to be fair and reasonable based on current vendor catalog pricing and minimum Government discounts per ECAT contract. Beacon Point, American Medical Depot and Firehouse Medical will be issued a Request for Quotation (RFQ) prior to the placement of the delivery order in an attempt to obtain better pricing than was previously negotiated. This acquisition represents the best value to the Government consistent with FAR 8.404(d) and will result in the lowest overall cost alternative to meet the Government's needs. Market research was performed on June 10, 2015 by searching this particular manufacturer's model number and by the item description [J1]. [BRADCCTS2] We also conducted market research using manufacturer information from websites and marketing/media material. There were no additional sources found that can meet the Government's requirements for this field sink unit. There are no additional facts supporting this justification. ECAT vendors have the opportunity to add products that fall within the scope of the ECAT contract at any time. DLA Troop Support will continue to attempt to locate other acceptable sources for the ECAT program for future requirements as they arise; however, any new items would need to meet METC's and AFMMAST requirements and undergo testing and evaluation.
