- (6) Attachment F DLA SP Camera Pass Request Form Revised Amendment 0001 Amendment 0001 is incorporated into solicitation SP3300-16-B-0007. Amendment 0001 is being issued to answer a vendor question. Question: I noticed in the attachments there is a camera pass request form. Does this need to be turned in prior to the site visit if they plan on bringing a camera and, if so would I turn that into you? Answer: If you plan to take pictures at the site visit the pass request form does need to be sent to joshua.woodworth@dla.mil NLT 0800 Friday 22 April 2016. The form must be completed except for signing the bottom. The form will be signed when picked up at Pass and ID at the front gate. The pass request form has been updated, (6) Attachment F DLA SP Camera Pass Request Form is replaced with the attached (6) Attachment F DLA SP Camera Pass Request Form Revised. Use the revised form if you plan to take photos. All other terms and conditions remain unchanged.
