- The Centers for Disease Control and Prevention is seeking a medical testing firm to provide fee-for-service medical testing when needed to provide blood/urine analysis, chest x-ray with b-read, foreign travel immunization and medication, stress test and other services that cannot be provided by CDC’s on-site health clinic in Cincinnati, Ohio. Vendor must be located within a 20-mile radius of the two CDC NIOSH headquarters buildings at 1150 Columbia Parkway and 5555 Ridge Avenue. We are planning on awarding this requirement as a Blanket Purchase Agreement, with costs to be paid on credit card so long as the price remains under the micropurchase limit of $3,500.00; otherwise invoices will be paid against BPA call through modification. Two option years are also being considered based on satisfactory performance at the end of the base year. Vendors must submit three separate bid lists (contained on the Statement of Work as Page 2) for each option year (August 1, 2016 – July 31, 2017, August 1, 2017 – July 31, 2018, August 1, 2018 – July 31, 2019). Please keep any yearly cost increases in mind when bidding on the option periods. See Statement of Work for complete details. This procurement is no longer set aside for Small Business. All qualified vendors can bid. Additional qualifications: You must be registered at SAM.gov to conduct business with the Federal government. The registration process typically takes several weeks, so if you wish to apply for this procurement and are not registered at SAM.gov, it is suggested that you start the process immediately. Your organization must be able to perform all tests required, not just a majority or some. Proposals which indicate otherwise will be considered not technically acceptable. Proposals which do not submit three separate bid lists for the base year and two option years will also be flagged. If you have questions about anything contained in the Statement of Work, please direct them to the Purchasing Agent at lml3@cdc.gov and they will be forwarded to the project officer/COR for response, which will be uploaded as an attachment to this announcement for use by all bidders. Questions should be submitted no later than April 27, 2016 by close of business (4:30 p.m. ET). Your proposal should be submitted by May 4, 2016, by close of business (4:30 p.m. EDT). Your proposal should include two documents, your technical proposal and your business proposal. If any labor rates are included, they should be clearly broken out (i.e. "Attending physician @ $50/hour X [number of hours]). Please address all segments of the Statement of Work and describe any methodology and procedures. Award shall be made to lowest bid technically acceptable, technical acceptability to be determined by the project officer/COR.
