Description
This is a combined request for quote for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-16-Q-0244 is issued as a request for quote (RFQ). The North American Industry Classification System (NAICS) Code 442110 Furniture Stores and a Business Size Standard of 500 employees are applicable. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-85. No telephone requests will be accepted. Quote is requested for the following: The Department of Veterans Affairs VA Illiana Health Care System 1900 E. Main Street Danville, IL 61832 is hereby soliciting offers to provide items listed below. We anticipate entering into a fixed price contract with the contractor meeting the Brand Name or Equal requirements and providing the best value to the Government. Items will be delivered to the VA Center location noted above. Please include warranty information and delivery/shipping terms and conditions and cost clearly on your quote. All quotes must provide the above items/options in as much detail as possible to assist the VA with determining the most advantageous selection. This award will be made based on the lowest price and best value. It is the responsibility of the offeror to demonstrate compatibility of items offered other than the part numbers described above. Selected vendor shall coordinate with Tonya Moore, VA Interior Designer at (217) 554-5596 to finalize all color selections prior to placing order in production. Provide 3D and single plan view drawings for all typical, to include corresponding parts list; evidencing compliance with all salient characteristics. For each type of exposed finish required, provide a sample, not less than three (3) inches square, of same thickness and material indicated for the work. Sample(s) shall be submitted within 10 business days of award. If finishes involve normal color and texture variations, include sample sets showing the full range of variations expected. Literature and specifications shall be submitted within 10 business days of award. Delivery to be completed as soon as possible after award. Substitution requests ("Or Equals") shall meet salient characteristics outlined. Substitution requests shall be similar to the specified dimensions, color scheme, function, and quality. "Or equal" products submissions shall include manufacturer's product brochure including specification information, warranty data, and physical samples of manufacturer's full range of finish options. All specified product(s) shall be under standard GSA Contract warranties. It is essential that furniture required be designed, procured, delivered, staged/moved, set-up and installed concurrently with the activation phase for government operational use no later than the expected dates of occupancy of the project. Anticipated furniture installation completion date to be 4/30/2016. The Government will not provide on-site storage for furniture requirements. Necessary storage must be included in proposal. Contractor shall be responsible for all aspects of furniture delivery, staging, installation/configuration, service, reconfiguration, and warranty service shall be provided in accordance with new construction space requirements that will result in a turn-key furniture solution in accordance with the final furniture design (subject to final working/construction drawings). Contractor shall check shipping documents with order documents to insure complete order was received. Installation must be performed by a licensed and bonded installation company, including any proprietary certifications necessary to maintain product warranties. Contractor shall have a minimum of three years of knowledge and experience with Herman Miller, Herman Miller Milcare, Steelcase 9000, Steelcase Wood, Teknion, Kimball Cetra, Kimball Wood, Artopex, Midmark Hospital products, (or approved equal), power and non-power chairs, with reference to installation of new, moving, cleaning, and servicing and inventory and storage of such products as directed by the COR. Contractor shall have a minimum of three years of experience in a Hospital setting and daily operations with reference to code situations and patients. Contractor shall have the ability to work with electrical, engineering, carpet, voice and data trades to coordinate timing and meet installation time lines as directed by COR. Contractor shall work along with the COR on project punch list and final completion of all projects. The Vendor shall furnish all supplies, equipment, fasteners, fixtures and services required to perform the service under this contract. The Vendor shall be responsible for any missing parts and components not included in order to carry out the installation. Vendor shall provide floor protection while working in all VA facilities. All material handling equipment shall have rubber wheels. Contractor shall be required to have necessary upholstery-cleaning equipment for cleaning of chair and panel fabric that will be directed the COR. Contractor is required to provide equipment and biodegradable-cleaning solutions as needed to perform the contract. The Vendor shall perform all work in accordance with this Performance Work Statement. Installing Vendor shall ensure all staff arrives on site with all necessary tools and equipment required for the installation including dollies, ladders, hand tools and carts to deliver and install the products as required to installation work site. All Vendor staff shall come to the job site equipped with all necessary safety gear including but not limited to eye protection, hard hats, etc. Any damage incurred through the delivery or installation process must be repaired within 30 days at the furniture vendor's expense. Deliveries must be scheduled through the VA interior designer before installation. All work is to be performed between 8:00am and 4:00pm weekdays, unless specifically coordinated otherwise. All work is to be performed in the newly constructed Green Homes, which does not have a loading dock. Contractor is required to supply all necessary equipment for in-moving including dollies, flat carts, jacks, panel carts and floor protections and boxes as directed by the COR. Contractor shall have hospital security clearance and is required to wear a hospital ID badge at all times. Vendor is responsible for registering with VA Police upon entrance and exit of the building. Vendor is responsible for notifying VA Interior Designer in advance of arrival and departure, providing review of all product and site conditions before sign off. There shall be no smoking, eating, or drinking inside the hospital at any time. Smoking in designated areas outside the buildings shall be allowed The Vendor shall conduct a joint inspection with the Contracting Officer's Representative (COR) once all Furniture has been installed. The COR Interior Designer shall inspect all phases of delivery and installation with a sign off of any and all missing or damaged products in the form of a Vendor provided punch list. Awarded Vendor shall provide detailed punch list items and established dates for replacement parts and/or short ship items from the manufacturer(s).The COR shall ensure all work is completed satisfactorily prior to acceptance. Disputes shall be resolved by the Contracting Officer. Upon award, the VA will require that the bill of materials is tagged by building and room for ease of installation. A project manager, project designer or installation foreman must be onsite during deliveries and installation. Awarded vendor shall include removal of all related shipping debris and construction cleanup associated with installation of the furniture. The Vendor shall be responsible for any damage to the building that occurs due to Vendor error or neglect. The Vendor shall be responsible for the coordination of staging areas and parking within the receiving area of the building. Floor Plans will be provided to the successful bidder in AutoCAD 2014 or PDF's. Upon completion of the project, two complete sets of documentation; Installation plan, Punch list, Operator manuals, Warranties, Maintenance and service manuals shall be provided.
