- 2016-Q-65011 Commercial Item Clauses Attachment This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is RFQ 2016-Q-65011 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: CLIN 0001: Total Price Statement of Work for Portable Ultrasound System Background : The Human Factors and Ergonomics Team (in the Research Branch) at the National Personal Protective Technology Laboratory requests the purchase of a portable ultrasound system to be used for monitoring and collecting data during human subject testing. Objective : Acquire a portable ultrasound system Requirements : The portable ultrasound system must satisfy the following requirements to be considered for acquisition. • The ultrasound system must be a portable and standalone system. • The system must include one general purpose transducer for vascular access such as carotid/brachial/femoral arteries. • The monitor size must range between 12 and 15 inches. • The total weight of the system (including a transducer) must not exceed 20 lbs. • The system must provide a color mode for blood velocity and flow measurements. • The system must provide 2D and M-mode imaging capabilities. • Additional information regarding (1) product release date, (2) warranty, and (3) training availability must be included. Schedule : Delivery Date - No later than 60 days after acceptance of PO. Your quote shall include the following: - Total Price for CLIN 0001 - Delivery Date: No Later Than Sixty (60) Days After Receipt of Order - Delivery Address: NIOSH - National Personal Protective Technology Laboratory 626 Cochrans Mill Road Building No. 166 - Receiving Room Pittsburgh PA 15236-3611 Acceptance and FOB Point: FOB Destination The quote format is at the discretion of the offeror. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-86 (January 01, 2016). Product description and specification information adequate for determination of the portable ultrasound system satisfying the requirements to be considered for the acquisition must be provided with the quotation. This acquisition is set aside for small businesses under North American Industry Classification System (NAICS) code 334510 and small business size standard of 500 employees. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management. To register apply via the Internet at http://www.sam.gov.
