- Pricing Sheet if needed Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotes (RFQ) #246-16-Q-0025. This procurement is a 100% small business set-aside under NAICS code 561710 with a standard business size of $11.0 million. Contractor shall provide a firm-fixed rate for pest control services at both the Clinton and Watonga Indian Health Center (pricing sheet attached). This requirement is a base plus 4 option year requirement. Estimated start date is June 1 2016. The closing date for receipt of quotes is May 13 2016 at 4:30 pm. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on May 13 2016. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. Qualified vendors will review the following and submit applicable information. No phone inquiries! STATEMENT OF WORK - ECOLAB PEST CONTROL SERVICES I. General: The contractor shall provide pest control services for both the interior and exterior of the Clinton Indian Health Center and the Watonga Indian Health Clinic. Pest shall include insects such as roaches, ants, scorpions, bed bugs, June bugs, locust and any other crawling or flying insect. Emergency calls shall also be written into the contract for removal of rodents, skunks, snakes and any other unwanted animals. II. Background: This is a Joint Commission requirement stated in EC.01.01.01. Standards for Ambulatory Care. III. Scope: The Contractor shall provide the following services for the Clinton and Watonga Indian Health Centers: • Watonga shall be internally treated once per month for each calendar month. • Watonga shall be externally treated once per month during the months of June, July and August only. • Clinton shall be internally treated once per month during the months of January, February, March, April, May, September, October, November and December only. • Clinton shall be internally treated twice (at least 15 days between treatments) during the months of June, July and August only. • Clinton shall be externally treated once per month during the months of June, July and August only. • Contractor shall use chemicals that are eco-friendly and that do not stain or create residue on exterior building structures or internal baseboard. • The contractor shall respond, upon request, for mitigation of pests on an as needed basis. The contractor shall provide all labor, transportation and equipment to perform the work that are separate from the internal and external maintenance treatments. • The contractor shall respond, upon request, for mitigation of non-insect pest such as rodents, skunks, snakes. The contractor shall provide all labor, transportation and equipment to perform the work that are separate from internal and external treatments. IV. Period of Performance: June 1, 2016 to May 31, 2017, base contract with 4 option renewal years. The Indian Health Services reserves the right to renew each option year. V. Contractor Qualification Requirements: The contractor is responsible for providing personnel with the necessary level of expertise to support the task activities and requirements in this SOW. Contractor shall provide two (2) references upon submission of quote, as well as two (2) past performance appraisals. The contractor shall provide a single point of contact that shall serve as the project manager for the life of the contract. VI. Special Terms and Conditions: • Contractor staff shall check-in with the facility point of contact and receive a visitor badge in order to conduct scope requirements. Contractor staff shall check-out upon completion of work. • Contractor shall provide SDS sheets for all chemicals used within this contract. VII. Schedule of Work: • Maintenance treatment applications as outlined within scope noted above. All work shall be conducted Monday - Friday between 8am to 5pm. • As needed treatment applications as outlined within scope noted above. All work shall be coordinated/approved by COR and may occur outside of normal business hours if required. VIII. Rates of Payment: The quotes for pursuant to solicitation shall be: • Internal and external maintenance treatment costs at Watonga Indian Health Center • Internal and external maintenance treatment costs at Clinton Indian Health Center • As needed service for mitigation of pests upon request at either Clinton or Watonga Indian Health Center. • As needed service for mitigation of non-pests upon request at either Clinton or Watonga Indian Health Center. The rates shall be payable upon receipt of full completion of scope requirements. Invoicing shall be completed as outlined in section XI. IX. Indemnity and Insurance: Contractor be licensed by the State of Oklahoma and shall possess liability insurance in accordance with industry standards. A copy of liability insurance shall be submitted upon quote submission. X. Government Furnished Information, Property and Services: The government will provide a point of contact to assist contractor in assuring all areas of the internal and external buildings are applied in accordance with scope. XI. Invoicing Requirements a. The Contractor shall be required to provide an original invoice to Oklahoma City Area Indian Health Service, Division of Financial Management, 701 Market Center Drive, OKC, 73114. b. Each invoice shall include the following information: i. Contractor's name and invoice date. ii. Contract number or authorization for delivery of services. iii. Description, price and quantity of services actually rendered. iv. Name, title, phone number, and complete mailing address of official to whom payment is to be sent. c. Invoice shall be submitted upon successful completion of scope requirements. XII.
