Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6- Streamline Procedures for Evaluation and Solicitation for Commercial Items, and is being conducted under FAR Subpart 13.5- Simplified Procedures for Certain Commercial Items apply. The solicitation number is VA256-16-Q-0617 and is issued as a Request for Quotation (RFQ) The proposed contract action is for PET Radiopharmaceuticals- Fluorodeoxyglucose (F18-FDG) 15mCi for which the Government intends to solicit competitive offers. Interested persons may submit proposals, and all eligible proposals received prior to the response date listed on FBO will be considered by the Government. There is no set-aside requirement for this procurement. The NAICS code is 325412 and the size standard is 750 Employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83). Only emailed requests received directly from the requester are acceptable. Delivery FOB Destination to Overton Brooks VAMC 9510 East Stoner Ave, Shreveport, LA 71101 RESPONSES ARE DUE TUESDAY, May 2nd, 2016, by 2:00 p.m. Central Standard Time (CST). Proposals are due electronically to veronica.maskell@va.gov no later than the response date listed on FBO. Only electronic proposals will be considered.
