Solicitation Deviation Security Attachment Technical Personnel Staff List Subcontracting plan Client Notification Letter Bank Notification Letter Business Management Questionaire Statement of Work April 2016- No Masters Required Cover Letter RFQ-CTS-058-16 CTS in Springfield, IL The Federal Bureau of Prisons, Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Springfield, Illinois. The location where services are to be provided should be accessible to public transportation, within 10 mile radius of the U.S. Post Office at 2105 East Cook St., Springfield, IL 62703. Performance periods under this contract will be for a one-year base period to begin on July 01, 2016 with nine (1) one-year option renewal periods with service ending (if all options are renewed) on May 31, 2026. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, Option Year Five, Option Year Six, Option Year Seven, Option Year Eight, and Option Year Nine respectively. The Following services are required and may be sub-contracted to another licensed contractor. The request for quote will be available on or after April 25, 2016 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions.
