- RFQ with SOW and Access Clause RFQ WX02560Y2016T ANTENNA INSTALLATION PROJECT This is a combined synopsis/solicitation for commercial items, prepared in accordance with FAR Subpart 12.6 and 13, as supplemented with additional information included in this notice. This requirement is also being solicited through a mandatory DHS Strategic Sourcing vehicle; however, if open market pricing is lower than the quotes received via the strategic sourcing RFQ, FEMA may award to an open market source. This announcement constitutes the only solicitation. Quotes are being requested and no other written solicitation will be issued. This solicitation is being issued as a Request for Quotation (RFQ). The RFQ clauses and provisions are those in effect through Federal Acquisition Circular 2005-87-1, effective 3/16/2016. The NAICS code for this requirement is 237130. The following FAR Clauses/Provisions apply to this requirement: 52.212-1, 52.212-2, 52.212-3, 52.201-10, 52.217-8, 52.222-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.2225-13, 52.232-33 Source selection will be based on a Low Price, Technically Acceptable (LPTA) basis. In order to be determined ACCEPTABLE overall, technical submissions must be rated ACCEPTABLE for each of the Technical Evaluation criteria. INFORMATION TO BE PROVIDED BY THE OFFEROR A. TECHNICAL SUBMISSIONS Offerors should submit a brief Technical Submission, not to exceed three (3) pages, addressing each of the Technical Evaluation Criteria shown below. Responses to each of the Technical Evaluation criteria will be rated either ACCEPTABLE or UNACCEPTABLE. 1. Schedule – Offerors should describe their proposed schedule to perform the work in accordance with the SOW. 2. Resources – Offerors should describe the resources they plan to utilize to perform the work in accordance with the SOW. 3. Past Performance – Offerors should provide telephone and e-mail contact information for three references for which they have performed similar work to that in the SOW. References will be contacted and asked to rate the quality of the offeror’s work on an ACCEPTABLE/UNACCEPTABLE basis. B. PRICING SUBMISSIONS Offerors should provide a firm fixed price quote to complete all of the requirements of the SOW. Offerors should show how the proposed pricing was determined, including a breakout of costs such as labor, equipment, travel, etc. This pricing information should be consistent with the Offeror’s Technical Submission, and will be used to evaluate price realism and reasonableness. OTHER INFORMATION PLACE OF PERFORMANCE: The place of performance for this effort will be at the Mt. Weather Emergency Operations Center (MWEOC), Mt. Weather, VA 20135. MWEOC is a restricted area and access is controlled. The attachment, entitled Deliveries and General Access, gives information and procedures for access to the site. Be advised that contractor personnel proposed to work on this requirement must be able to meet the security requirements for access to the MWEOC facility. No Site Visit is planned for this RFQ. SAM REGISTRATION/OFFEROR REPRESENTATIONS AND CERTIFICATIONS: Prospective Offerors must be registered in the SAM database prior to award. Information on registration may be obtained electronically at https://www.sam.gov/portal/public/sam/ or by calling 1-866-606-8220. If currently registered in SAM, please state in your submission and provide a DUNS number. If the Offeror has not completed FAR 52.212-3, Offeror Representations and Certifications –Commercial Items in SAM, the Offeror should include a completed copy of that provision with their quote. Questions regarding this requirement shall be directed electronically to Kristin.Wright@fema.dhs.gov. DELIVERIES AND GENERAL ACCESS TO DHS, FEMA, MOUNT WEATHER EMERGENCY OPERATIONS CENTER (MWEOC) I. General A. Classified material and areas exist at the Mount Weather Emergency Operations Center (MWEOC) that must be protected in the interest of national security. The facility is a restricted area. Access to the MWEOC is only provided to those persons having official business related to the mission and operations of the MWEOC. The Federal Emergency Management Agency (FEMA) Administrator, the MWEOC Executive Administrator, or their designee must approve all persons and vehicles entering the MWEOC. All persons must register with the MWEOC Security Services Branch and must receive a MWEOC identification badge or other approved credentials to enter or remain on the premises. B. Access is limited to those persons who are United States of America (U.S.) citizens or have been granted an exception by the Executor Administrator, or his/her designee and vetted through the DHS Foreign National Visitor Access Process. Sponsors of personnel seeking access must be U.S. Government employees. C. All vehicles, packages, handbags, briefcases, and other containers transiting in or out of the facility are subject to inspection by MWEOC law enforcement. The Executive Administrator authorizes inspections at the MWEOC to prevent the possession and use of items prohibited by these rules and regulations or by other applicable laws, to prevent theft of property and to prevent the wrongful obtaining of classified information under 18 U.S.C. 793. If individuals object to such inspections, they must tell the officer on duty at the access point prior to entering the MWEOC. Individuals who refuse an inspection of their vehicle or possessions will be denied entry. II. Procedures A. Access may be granted to the following Personnel: 1. DHS/FEMA employees 2. Tenants or other permanently assigned employees 3. Visitors on official Government business 4. Contractor employees or vendors, including delivery personnel B. Access is controlled at the Access Control Point Visitor's Center, Building 911. Building 911 is staffed to assist personnel assigned to or visiting the MWEOC in gaining access on a limited or intermittent basis, issue badges to those authorized a permanent badge, and resolve badging problems. C. Per the requirements of Homeland Security Presidential Directive 12 (HPSD-12), the MWEOC will grant unescorted access on the basis of a secure and reliable form of identification that can be authenticated electronically. D. All visitors and guests will be screened to determine if they meet eligibility requirements for access into a FEMA space. E. All visitors and guests must be sponsored by an approved MWEOC sponsor before they are granted access to the facility. F. All individuals entering the MWEOC must be properly identified and in possession of a valid credential or access badge. G. While inside the MWEOC, individuals must at all times conspicuously display their credentials/access badge between their neck and waist. H. Individuals must secure their FEMA identity credential or access badge from view upon exiting the facility. I. Law enforcement officials may retain custody of their firearms while visiting the MWEOC in the performance of their official duties upon proper display of verifiable law enforcement credentials. J. The FEMA OCSO must first qualify any foreign national requesting access before access is granted by the MWEOC Executive Administrator. The federal government office sponsoring the foreign national must submit their request to the FEMA OCSO at least 30 days in advance of the proposed visit date. K. Individuals must surrender their identity credentials/access badge to Mount Weather Police or Security Specialists upon request. L. Individuals must return their FEMA identity credential/access badge and building pass when access is no longer required or authorized, such as resulting from suspension, termination of employment or resignation. MWEOC visitor badges must be returned at the conclusion of the visit. M. Contractors must submit all access requests through their sponsoring Government Points of Contact. Access requests must meet criteria as defined in 44CFR Part 15, Subpart A, and HSPD-12. N. All federal employees, contractors, vendors and delivery drivers will be required to follow identification standards outlined in the Department of Homeland Security REAL ID Act - Title II http://www.dhs.gov/real-id-enforcement-brief. O. Proof of citizenship may be requested for persons born in a foreign country. When required, original or certified forms must be presented. Acceptable forms are; Certification of Birth abroad issued by the Department of State, Naturalization Certificate, U.S. Citizen ID Card, or U.S. issued Passport. P. Request for access must be provided to the MWEOC Area A Access office twenty-four (24) hours in advance of all deliveries. This request can be made by submitting a properly completed Mount Weather Access Application. Deliveries to the warehouse receiving area are authorized between the hours of 0700 - 1500 Monday - Friday, with the exception of federal holidays. Deliveries requested at other times must be arranged (by the requester of delivery) directly with warehouse personnel (540-542-2072) and will depend on the availability of the MWEOC staff. III. Requesting Access A. Day and Emergency Visitors, sponsored vendors, contractors and delivery drivers. 1. Contracting Officers/Specialist (CO/CS), Contracting Officer Representatives (COR) or Government Points of Contact (GPOC) will provide notification of all individual visitors and completed request forms to the MWEOC Area A Access office by submitting a properly completed Mount Weather Access Application twenty-four (24) hours in advance of desired arrival (for non-emergency events). 2. Completed Mount Weather Access Applications should be sent to the appropriate CO/CS, COR or GPOC with access signing authority for signature and submission to the Mount Weather Security Office. Mount Weather Access Applications may be emailed as long as they are password protected to safeguard Personal Identification Information (PII); the password must also be provided in a separate email or by phone. 3. A contractor, vendor or delivery driver possessing a valid Federal Government credential may be granted access for one year or another specific, shortened period of time, if requested. Annotation of credential possession should be made at the top of the Mount Weather Access Application prior to submission. 4. A contractor, vendor or delivery driver not possessing a Federal Government credential will be processed through the MWEOC established National Crime Information Center (NCIC) security screening. 5. The MWEOC Access Office will perform a NCIC check to screen vendors or delivery drivers requiring access to the facility but no access to Government IT systems. 6. If the NCIC check indicates the applicant meets FEMA established "Pre-Employment Screening Guidance for Suitability" for temporary access, escorted access will be granted immediately. a. Any Government, guest or contractor approved for unescorted access can escort individuals on site. b. Escorts must remain in control of any individuals they are escorting. 7. Any NCIC check which does not meet the "Pre-Employment Screening Guidance for Suitability" for temporary access is cause for immediate denial of access. Notification will be made to CO/CS, COR or GPOC of the denial. 8. The sponsoring CO/CS, COR or GPOC will decide if it is in the best interest of the government to offer litigation to the denied applicant. B. No guests or visitors will be billeted at MWEOC unless they have been granted unescorted access C. Facility access requirements may change with little or no notice based on changes to the prevailing threat situation.
