- FA7014-16-R-5007 Executive Leadership Training Solicitation Attachment 6 - Pricing Worksheet Attachment 5 - Past Performance Questionnaire Attachment 4 - Past Performance Cover Letter Attachment 3 - Volume IV Past Performance Reference Form JB Andrews Visitor Center Pass Form FA7014-16-R-5007 Quality Assurance Surveillance Plan (QASP) The Air Force District of Washington plans to fill a training support requirement for Executive Leadership Training. This effort will be performed in support of AF CPI training, transformation, and efficiency initiatives sponsored by the Air Force Deputy Chief Management Officer (DCMO) under Secretary of the Air Force (SAF), Office of Business Transformation (MG). The Execs Training services will require the contractor to provide support that will include integrated lodging, dining facilities, course materials and hands on review of one or more private sector process improvement application(s) as an industry tour. Classrooms shall accommodate 25 executive attendees plus two (2) Government observers and will be two full academic days. The resulting contract will be unrestricted under NAICS 611310 (Colleges, Universities and Professional Services) and is expected to be a three year (Base year and two option years) single Firm Fixed Priced award. It is the responsibility of those responding to the solicitation to obtain all amendments and/or other applicable documents. Questions on the actual requirement, tasks or the CPI Course shall be submitted to the Contracting Officer, Mr. Bumjoon.J. Pak at bumjoon.j.pak.civ@mail.mil and Contract Specialist, Ms. Cynthia I. Harper at cynthia.i.harper.ctr@mail.mil by noon (12:00 PM Eastern Time) on 29 April 2016. The Government intends to provide responses via solicitation amendment on www.fbo.gov NLT noon (12:00 PM Eastern Time) on 2 May 2016. No telephone inquiries will be accepted. The Offeror shall submit Volumes I through V IAW Addendum 52.212-1 Instructions to Offerors, Commerical Items. Proposals are due by noon (12:00 PM EST) on 24 May 2016. It is the potential offeror's responsibility to monitor the FBO website on a regular basis for any updates. Other documents, as necessary, will be posted when available.
