- Blanket Purchase Request for Structure Demolition Kentucky Counties: Harlan, Knox, Bell, & Whitley This Sources Sought Notice will be utilized by the U.S. Army Corps of Engineers, Nashville District to establish a list of sources interested in Blanket Purchase Agreements (BPA) for Structure Demolition Services in the Kentucky Counties of Harlan, Knox, Bell, and Whitley. A Blanket Purchase Agreement (BPA) as defined by FAR 13.303(a) is a simplified method of filling anticipated repetitive needs for supplies or services by establishing " charge accounts " with qualified sources of supply. The BPA does not guarantee, nor obligate the Government to, any orders to a BPA holder in any way. The North American Industry Classification System (NAICS) Code for this BPA is 238910 and the SBA Size Standard is $15.0 Million. The agreements will have a period of performance of five (5) years from the date the Master Agreement is established. As services are required, the Government will issue a Request for Quote (RFQ) to vendors actively holding a BPA Master Agreement. BPA holders will not be required to quote on every RFQ. Quotes will be required to be submitted electronically, via email, or fax. Interested Vendors should be capable of providing structure demolition services in the above listed Counties of various types as listed in the General Requirements. To be eligible for a BPA, an Interested Vendor SHALL have an active record in the System for Award Management (SAM), at www.sam.gov. Your participation in this matter would be greatly appreciated. Please do not respond after the close date listed below. Please provide your responses by 3 May 2016. GENERAL REQUIREMENTS Registration in System for Award Management STATEMENT OF WORK Furnishing all plant, labor, materials, and supplies to perform all work required to demolish and dispose of debris from structure demolition in accordance with all applicable federal, state and local EPA/OSHA regulations. Additional requirements to be furnished as needed. Agreement may be terminated at any time by the Contracting Officer or Vendor upon written notice. 1. No wall or part thereof shall be permitted to fall outwardly from any building except through chutes or by other controlled means or methods, which will ensure safety and minimize dust, noise and other nuisance. Any part of a building which has become unstable through removal of other parts shall be removed as soon as practicable and no such unstable part shall be left free-standing or inadequately braced against all reasonably possible causes of collapse at the end of any day's work. 2. All footings and foundations shall be completely removed from the site unless specifically allowed to remain by written letter from the Judge-Executive. 3. All basement floors are to be broken up and backfilled. The contractor shall ensure that no basement excavation will remain open and exposed for more than 24 hours. 4. The contractor shall remove all concrete slabs, asphalt, surface obstructions, masonry slabs and appurtenances. 5. Fences shall be completely removed from the site, except fences on the apparent boundary between a contract parcel and an improved non-contract parcel shall not be removed. All posts for support shall be pulled out or dug up so as to be entirely removed. 6. All piping, posts, reinforcing bars, anchor bolts, railings and all other partially buried objects protruding from the ground shall be removed. The remaining void shall be filled with soil and compacted in accordance with these specifications. 7. The contractor shall remove all dead trees, stumps, brush and weeds, whether standing or fallen. The contractor shall protect all trees not removed from damage by the demolition operation. 8. Fuel tanks, above or below ground shall be carefully removed and disposed of in a safe manner in accordance with the State Fire Marshal's regulations. 9. Outdoor toilets and septic tanks shall be pumped out by a licensed company. The toilet building or septic tank shall be demolished and the pit shall be backfilled. 10. Cisterns and meter pits shall be demolished and removed. The excavations shall be backfilled and compacted. 11. All wells shall be plugged and abandoned in accordance with the State Code. A Kentucky Department of Natural Resources Abandoned Water Well Plugging Record shall be filed upon completion of the well abandonment. 12. All debris shall be delivered to a landfill and tickets identifying the landfill shall be turned in with invoice. No burning on site. 13. Handling of asbestos material is subject to all applicable state and federal regulations. 14. The handling of Freon containing appliances is subject to all applicable state and federal mandates and regulations. The contractor shall be responsible for the identification and removal and disposal of the material in accordance with applicable regulations. All costs shall be incidental and shall be included in the lump sum bid for demolition. 15. The site shall be graded to and shaped to drain and shall not permit ponding of water. 16. All sanitary sewer services and water services shall be disconnected and plugged in conformance with local regulations. 17. All disturbed areas associated with the work shall be seeded, mulched with straw, and cared for until a suitable stand of grass grows. 18. The contractor shall comply with all applicable current federal, state and local safety and health regulations. 19. The contractor shall furnish and place a safety fence around the site of the work adequate to secure the demolition site, including any resulting debris or excavation, and to prevent pedestrian access. The fencing, including all materials shall be considered incidental to the demolition. The safety fence shall remain in place until the demolished materials are removed from the site and all holes or excavated areas are backfilled. The fencing material shall remain the property of the contractor. 20. Only the contractor and its employees are allowed to demolish, dismantle, detach or dispose of any part of the demolition structure or its contents. Once demolition of a structure has started; it will be completed within 5 days. 21. At the end of each workday, the contractor shall clean sidewalks, streets, and private property of any debris caused by the demolition operation. 22. Work shall begin within 10 days after BPA call is issued.
