- Pricing Schedule RFP - LCISS16R0047 -First Aid -CPR/AED Training Combine Synopsis/Solicitation Cover Page FBO Solicitation Summary for Combine Synopsis/Solicitation for Library of Congress CPR/AED and First Aid Training (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is LCISS16R0047 and issued as a Request for Proposal (RFP) (iii) This solicitation document incorporates all mandatory commercial item provisions and clauses currently in effect in the latest Federal Acquisition Circular (FAC) 2005-81. (iv) Offerors shall provide pricing for all the deliverables as outlined in the attached RFP. (v) The Health Services Office (HSO) at the Library of Congress is charged with promoting and maintaining optimal health, wellness and productivity of all Library staff. The Office offers a wide spectrum of occupational health programs and services, including medical emergency response. The Health Services Office provides required Cardiopulmonary Resuscitation (CPR) /Automatic External Defibrillator (AED) training for Medical Emergency Response Team (MERT) personnel and Health Services staff. To facilitate a prompt response to medical emergencies, CPR/AED training is offered to all Library employees. First Aid instruction is provided to MERT personnel and interested Library employees. The attached RFP, contains the requirements to be satisfied by the successful offeror awarded a contract as a result of this solicitation. (vi) HSO anticipates a Single Award IDIQ with Firm Fixed-Price (FFP) Orders to obtain the services. The contract‘s period of performance is anticipated to be: May 09, 2016 through May 08, 2021 The Option Periods under the IDIQ will be as follows: Base Year Period: May 09, 2016 through May 08, 2017 Option Period I: May 09, 2017 through May 08, 2018 Option Period II: May 09, 2018 through May 08, 2019 Option Period III: May 09, 2019 through May 08, 2020 Option Period IV: May 09, 2020 through May 08, 2021 (vii) The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Refer to attached "Terms and Conditions" for additional proposal submission instructions. (viii) The provision 52.212-2, Evaluation-Commercial Items applies to this acquisition. Refer to attached "Terms and Conditions" for evaluation factors. (ix) Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror representations and Certifications-Commercial Items with their offers. (x) The clause at 52.212-4, Contract Terms and Conditions-Commercial items, as modified by the Library, applies to this acquisition. Offerors shall refer to clause 52.212-4 listed in attached "Terms and Conditions" for additional clauses applicable to this acquisition. (xi) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial items as modified by the Library, applies to this acquisition. Offerors shall refer to clause 52.212-5 listed in attached "Terms and Conditions" for additional clauses applicable to this acquisition. (xii) Additional contract requirements, terms and conditions are determined by the Contracting Officer to be necessary for this acquisition and consistent with customary commercial practices. The following attachments are included and are hereby incorporated into this solicitation: (1) SOW, and (2) Terms and Conditions. (xiii) Questions concerning any areas which, in the offeror's opinion, require clarification or correction must be submitted in writing to Zaneta Watson with a cc: to Andrew Sullivan via email (zawa@loc.gov and ansu@loc.gov) by 12:00 NOON EST on Thursday April 28, 2016. Answers to questions of a substantive nature will be provided to all offerors. (xiv) Proposals are due by 12:00 Noon EST on May 02, 2016. There are no exceptions to the time and date the proposals are due. Any proposal submitted after this time will not be considered. Submit via email to zawa@loc.gov with a cc to ansu@loc.gov. It is the offerors responsibility to ensure that the proposals are submitted timely.
