- Amendment 002 Solicitation Document. Amendment 002 Form. Amendment 002: The purpose of Amendment 002 to Solicitation 5A-16-A-0001 is to correct an error within the Total Evaluated Price Calculation, located within the Price Analysis section of Provision 4-2 Evaluation, with changes highlighted in light blue. This correction will provide an accurate representation of the Total Evaluated Price Calculation as it is populated in the Attachment C - Pricing document. All other terms and conditions of this Solicitation remain unchanged.
