STATEMENT OF WORK (SOW) Rheumatoid Arthritis Blood Testing Northport VA Medical Center, Northport NY (632) Pathology & Laboratory Medicine Service (113) Microbiology Department 1. INTRODUCTION AND OVERVIEW This statement of work will be issued under VA contract for VectraDA disease activity test. VectraDa is the first multi-biomarker blood test for rheumatoid arthritis disease activity. This emergency acquisition is needed due to the large uptick in number of tests and the current contract not being able to handle it. 2. BACKGROUND This test will give providers a disease activity score from 1 to 100 to assess rheumatoid arthritis disease in veterans thereby enabling the provider to treat the veteran suffering from RA. VectraDA measures the concentrations of 12 serum proteins. An algorithm is applied to these concentrations to calculate a quantitative disease activity score ranging from 1 to 100. Test results are intended to aid in the assessment of disease activity in Rheumatoid Arthritis patients when used in conjunction with standard clinical assessment. 2.1 SCOPE OF WORK The VectraDA disease activity test blood samples will be sent from the Northport VA Medical Center, Pathology & Laboratory Medicine Service, to vendor for testing. No more than 60 samples will be tested. 3. OBJECTIVES To provide VectraDA disease activity test to assess rheumatoid arthritis disease in veterans thereby enabling the provider to treat the veteran suffering from RA. 3.1 PERIOD OF PERFORMANCE "Not to exceed 60 tests for 5/1/2016-10/1/2016." 3.2 PROJECTED COST Please provide individual and total sampling pricing. 3.3 REQUIREMENTS Vendors to provide collection kit: 1. Polar-Pack Foam Bricks 2. SST collection tube with biohazard bag 3. Transport cooler and mailer. 4. Final result via Vectra-View internet reports, to Beth.LeMaitre@va.gov and faxed reports to 631-266-6015 attention Beth LeMaitre, Microbiology Supervisor. 3.4 Tasks The contractor shall conduct the following tasks: 1. Provide collection kits. 2. Provide final reports via Vectra-View internet reports to Beth.LeMaitre@va.gov and faxed reports to 631-266-6015 attention Beth LeMaitre, Microbiology Supervisor. All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM) located on the web at www.sam.gov. It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to this announcement must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at https://www.vip.vetbiz.gov/.
