Description

PWS Evaluator The Navajo Area Indian Health Services, Pinon Health Center (PHC) is requesting for proposals for interested offerors to provide Evaluating Services at Pinon Health Center and its surrounding communities. Period of Performance: 5/01/2016 through 08/31/2016. SUBMITTAL To be considered for award submit the following documents to the Contracting Officer via email: -Pricing Schedule and Product Specification form (must be signed) -Two letters of reference to provide evidence of past performance on contracts of comparable scope and complexity within the past five (5) years. -Capability Statement providing evidence of Clinical Evaluator Services, Logistics and Capabilities. (must be signed) EVALUATION FACTORS The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factor will be considered: Past Performance, Clinical Evaluator Services, Logistics and Capabilities and On-Site services. The evaluation factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value. Due to the urgent need the Government encourages interested offerors to submit early.
