- Attachment 3 Past Performance Questionnaire - 24 May, 2016 Suspense Ammendment 01 is to change the Past Performance Questionnaire due date from 4 May, 2016 to 24 May, 2016. SEE ATTACHED DOCUMENTS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The Combined Synopsis/Solicitation number is W912D0-16-R-0001 and incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-87, dated 06 Apr 2016. The NAICS Code is 561720 with a size standard of $18M. The government intends to award a firm-fixed price Requirements Contract for Custodial Services for the Directorate of Public Works, Fort Wainwright, Alaska. This procurement is 100% set aside for Small Business firms. Service period of performance (POP) will be a 12-month base plus four (4) 12-month options.
