- This is the Air Force Form 3000. Use this form to submit the types of materials you will use to complete the project (This document is only being provided for informational purposes at this point. Do not submit this form with your quote) This #2 of two (2) drawings documents. These will be used to complete the work. Please study them before providing your quote. The contractor shall provide all material, tools, equipment, transportation, supervision, management, and labor necessary to remove existing concrete rubble, construct concrete pads, footers, columns, bond beams and associated structures contained in the approved project plans in attachment #1, hereinafter referred to as "drawings". In the event of any inconsistencies, details provided in the drawings take precedence over any references within the Statement of Work (SOW). Contractor shall perform all work in accordance with this SOW and the drawings. Contractor shall coordinate all work with the Contracting Officer (CO) and is subject to the scheduling and phasing of work as stated in section 3.0 of this document. The project will consist of 3 phases. Phase 1 consists of clearing the existing concrete rubble, building up the grade to a higher elevation by 6 inches, and placing a 90ft x 25ft-4in pad, footers, columns and bond beams for the schoolhouse in accordance with the drawings. Phase 2 consists of placing a 8ft x 12ft pad, footers, columns and bond beams 15ft adjacent to the previous one for the comfort room in accordance with the drawings. Phase 3 consists of installing 1-2 horizontal holding tanks for sewage in accordance with paragraph 7 of this SOW, not the drawings. All work shall be performed at the Floridablanca Elementary School, Floridablanca, Pampanga, Philippines. All work shall be approved by the Contracting Officer prior to issuance of the Notice to Proceed (NTP) for the next phase.
