- Atch. 6 Experience and Past Performance Questionnaire Atch. 5 Wage Determination 2005-0047, Rev 13, 29 Dec 2015 Atch. 4 Alaska Tidelands Survey No 1564 Atch. 3 Vicinity Map Atch. 2 Appendix 4 Bark Monitoring Survey Form (2 optional formats) Atch. 1 Pre-Discharge Schedule of Items and Description of Requirement The US Forest Service, Region 10 contracting office in Anchorage, Alaska intends to post a request for quote (RFQ) for dive monitoring and reporting services to perform pre-discharge dive and create a report to Alaska Department of Environmental Conversation standards. Specific contract line items are identified in the combined synopsis solicitation request for quote. Place of contract performance is Tongass National Forest, Petersburg Ranger District, Rowan Bay Facility. Period of performance will begin with issuance of the notice to proceed with desired completion date of 31 May 2016. This is 100% set aside for small business. The NAICS Code is 561990 and the size standard is $11 million. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested based on the requirements outlined in this combined synopsis/solicitation. A written solicitation will not be issued. This solicitation, AG-0120-S-16-0018 is issued as a Request for Quotation (RFQ), for a firm fixed price type purchase order. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87, effective 06 April 2016. This requirement is a total (100%) small businesses set aside. The associated NAICS code is 561990 and small business size standard is $11 million. Attachments: 1.
