THIS RFI IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH FAR 15.201(e). PRICING SHALL ONLY BE USED FOR PLANNING PURPOSES AND SHALL NOT BE USED AS AN EVALUATION AGAINST ANY POTENTIAL OFFEROR SHOULD A SOLICITATION ARISE FROM THIS RFI. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification, hubzone, 8(a), small, small disadvantage, veteran owned small business or service disabled veteran owned small business) relative to NAICS 621399. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources to provide on-site respirator fit testing, training and certification to all Veterans Affairs (VA) employees participating in the Occupational Safety & Health Administration (OSHA) Respiratory Protection Program services for VA Healthcare Systems (VAHCS) at: San Diego, Long Beach, Greater Los Angeles, and Loma Linda. The information identified above is intended to be descriptive, not restrictive and to indicate the quality and services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the services that fulfill the required specifications. If a solicitation is published, a site walk thru may be available in order to address key details. If you are interested, and are capable of providing the sought out services please provide the requested information as well as the information indicated below. Statement of Work (attached below): Performance Work Statement (PWS) VISN 22 Respirator Fit Testing Contract 1. SCOPE OF WORK: This contract is to provide on-site respirator fit testing, training and certification to all Veterans Affairs (VA) employees participating in the Occupational Safety & Health Administration (OSHA) Respiratory Protection Program at below VA facilities. Fit testing and training shall be performed by using OSHA-approved testing procedures and media and shall include supplies and labor to perform scheduled services Monday through Friday, excluding national holidays (see 12). Services shall be scheduled and coordinated in advance with each Facility. All services are to be furnished at the VA Facilities locations listed below for the base period of 6 months with the Government's option to extend contract services for four (4) one-year option periods. (b) The contractor must have staffed personnel which can demonstrate formal training and or education in the OSHA compliant respirator fitting test (list all training, education, licenses/or certificates in your capability statement). (c) Contractor will collect the medical evaluation screening form, review it for completeness, and ensure a clinician (Occupational health) has approved their use of a respirator prior to the fit testing and training of the VA employee to ensure that this information is collected and that the employee is physically capable of wearing a respirator. Contractor will not perform fit tests to individuals who do not have an approved medical evaluation form. Note this requirement may vary at facilities throughout VISN 22. Contractor will assist in the selection of the appropriate respiratory protection and cartridge, if required, for each task evaluated (the medical evaluation screening form becomes the property of the VA medical facility). (d) Provide general awareness and OSHA compliant respirator protection training at the time of fit testing, on the proper fit and capabilities of the Government-owned respirator, limitations of its use, how to inspect it, protection factors, proper storage, and when to replace the respirator. Note: for N-95 filtering face-pieces used for airborne infectious disease; replacement schedule shall comply with facility specific infectious disease policies. This information will be obtained from facility specific COR. Additionally, training may require adaptation to comply with facility specific policies and practices. Contractor will submit training syllabus/materials to facility COR for evaluation, relevant updates, and approval. This approved training shall be used to conduct the general awareness & OSHA respiratory protection training. (e)Provide supplies (other than respiratory masks) to perform either or both qualitative and quantitative fit testing to N-95 disposable respirators, half face elastomeric respirators, and full face elastomeric respirators in compliance with Federal and California OSHA's Respiratory Fit Testing Procedures; using OSHA-approved testing media & protocols. VA Long Beach Healthcare System: Qualitative VA Loma Linda Healthcare System: Qualitative VA Greater Los Angeles Healthcare System: Qualitative for N95 only, Quantitative for half/full face VA San Diego Healthcare System: Qualitative for N95 only, Quantitative for half/full face (f) Provide training on use and maintenance of Powered Air Purifying Respirators (PAPR) and hoods. Employees who cannot be fit tested for a respirator will be trained on a suitable power air purifying respirator (PAPR) and hood. Note that all training pertaining to PAPRs shall comply with the requirements of section 2, paragraph (b). (g) Provide a laminated wallet size card that certifies their successful completion, to each employee that completes the training and fit testing. At the time of testing and training completion, the card, including employee name, medical evaluation release, assigned respirator, and training dates, will be provided. (h) Document all fit tests and training on a Respirator Program Record form provided by Contractor. The forms will be submitted to the VA medical facility COR for file, at the completion of training. (i) Input and track employee training and fit testing information in OHRS. This database must be kept current and updated at least every two weeks. Contractor must provide a monthly written update of yearly fit testing/training progress and submit it to each corresponding VA facility COR along with the goals, which shall be approved by the VA Industrial Hygienist, for the following month. A final summary report shall be provided to each individual VA medical facility at the end of each contract period that will show the results of the training. shall include medical evaluation screening forms to be provided to the VA employees at the time of fit testing and training; collection of the form; and, review for completeness of the form by the Contractor. The Contractor is to maintain and track records of VA employees that have been trained through a Contractor/VA medical facility electronic relational database. (j) Complete courses for SCBA and PAPR, safety awareness, respiratory protection, etc. for various departments. (k) Contractor shall send a copy of the Invoice to the COR for each facility prior to submitting the invoice electronically for payment. II. SAFETY REQUIREMENTS: All services provided shall be in compliance with Federal and California OSHA's Respirator Protection standard and be conducted in accordance with 29 CFR, Part 1910.134 Respiratory Protection, guidance published by the National Institute for Occupational Safety and Health (NIOSH), and the American National Standards Institute (ANSI). (a) The Contractor will ensure 100% compliance guarantee with Respiratory Protection Standards for all fit testing. (b) The Contractor will perform work in strict accordance with existing relevant, accredited standards and codes in order to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort will be made to safeguard human life and property. (c) Fit test hoods shall be sanitized after each use. 3. Locations are to be coordinated in advance with the COR and/or department's Supervisor-on-duty as specified by each facility COR. The work is to take place at the locations listed on the VA Facilities List. Upon completion of fit testing; room shall be restored to previous condition to include furniture, generated trash, and other devices that are relocated to successfully complete fit testing. All areas shall be maintained in a clean and presentable manner at all times. Due to OSHA strict requirements, contractor is required to comply with 100% FIT test completion rate for all types of respirators at each campus by September 30 of each year. Should VA scheduling conflicts result in less than 100% completion, Contractor will provide a list of required employees not fit tested will be required along with a plan of how to accomplish this fit testing by the end of each calendar year, reason for noncompliance, and corrective action to prevent noncompliance. 5. DELAY IN PERFORMING CONTRACT SERVICES: Contractor should fully understand that the participating VA employees are responsible for providing patient care, thus making advance scheduling a challenge. Contractor shall work closely with the designated COR and staff of the VA to ensure that scheduling is utilized for optimum results. Documentation of delays to provide contract services shall be provided to the COR for a concurring signature from the COR. Contractor's personnel are responsible for proper documentation of delays. The document must state the scheduled time that services were required, the reason for the delay, and the actual time that contract services could be provided. 6. TEST EQUIPMENT: The Veterans Affairs Medical Center will not furnish parts and/or test equipment (other than respirator masks) for the performance of this contract. It is the responsibility of the Contractor to bring the appropriate equipment and/or supplies necessary to complete the work as required. 7. CONTRACTOR'S RESPONSIBILITIES: The Contractor shall obtain all necessary licenses and/or permits required to perform the work. He/she shall take all precautions necessary to protect people and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, and others as well as any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. The Contractor shall maintain personal liability and property damage insurance prescribed by the law of the Federal Government. The Government reserves the right to accept or reject the Contractor's staff rendering of services. Complaints concerning the Contractor's personnel's performance of conduct shall be with by the Contractor, Contracting Officer's Representative with the final decision made by the Contracting Officer. 8. BADGES: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA grounds. I.D. badges must have an identification picture, and will state the name of the individual and the company they represent. Individuals not having their own acceptable identification will wear a visitor identification badge provided by the VA. These badges are to be signed for each day at the Police and Security Office and turned in each day upon leaving. 9. NORMAL WORK HOURS: Normal work hours for the purpose of this contract are between the hours of 6:00 a.m. and 6:00 p.m., Monday through Friday, excluding national holidays. 10.OTHER THAN NORMAL WORK HOURS: On rare occasions it may become necessary for the Contractor's personnel to provide contract services during other than normal work hours, weekends, or national holidays. Emergency services provided during these hours will be paid for under a separate, pre-authorized obligation number at the rates specified in "Supplies or Services and Prices/Costs". Authorization shall be obtained from the Contracting Officer or the Contracting Officer's Representative (COR) in advance for any work performed outside of normal working hours. If this authorization is given for the convenience of the Contractor, there will be no additional charge for labor, travel time, or other expenses. The Contractor shall provide emergency service on an as required basis. Emergency service shall be considered as service calls in addition to the scheduled work. This emergency service shall be provided as often as needed on a 24-hour basis, weekends, and legal holidays included. The Contractor shall be capable of responding to an emergency situation within two (2) hours after the notification of an issue. 11. SMOKING POLICY: Smoking is permitted only in designated areas on VA grounds and is strictly prohibited within VA buildings. 13.CELLULAR PHONES: Cellular phones will be turned off prior to entering the VA facility and are prohibited for use inside the building unless in case of an emergency. 14.PARKING AND VIOLATIONS: It is the responsibility of the Contractor to park in the appropriate designed parking areas; information on the parking is available from the COR or VA Police. The Government will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of government regulations may result in citations answerable in the United States (Federal) District Court. 15. CONTRACTING OFFICER REPRESENTATIVE (COR): Delegation of Authority letter(s) will be forwarded to the using service and Contractor after agreement has been signed, identifying the individual(s) as the COR(s). All authorized representatives of the Veterans Affairs Medical Center to order services and to accept or reject services performed will be delegated as COR(s). This designation will be furnished in writing to the Contractor prior to commencement of the contract. No service will be provided without the approval of the COR(s) or his/her designee of this contract. 16. CONTRACTOR POINT OF CONTACT: The Contractor will designate one (1) employee as the Point of Contact (POC) responsible for administrative matters in the performance of services under this contract. The POC will have full authority to act for the Contractor on all matters relating to the daily performance of this contract. An alternate may be designated, but the Contractor will notify the Contracting Officer and COR in writing those times when the alternate will act as the POC.
