- Address: Combined Synopsis Solicitation – Carestream Industrex HPX-1 Computed Radiography (CR) System, Response Date: 10 May 2016 at 1300 Hours, Mountain Standard Time (MST), Solicitation No: W9124R-16-T-0041, This is a Combined Synopsis/Solicitation (CSS) for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued., The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Fac 2005-87, Effective 06 Apr 2016 and Defense Federal Regulation Supplement (DFARS), current to DPN 20160325 (Effective 25 March 2016) Edition. Offerors that fail to furnish required representation or technical information as required by FAR provisions 52.212-1 or reject the terms and conditions of this solicitation may be excluded from consideration. It is the responsibility of the contractor to be familiar with the applicable provisions and clauses which can be accessed in full text at www.farsite.hill.af.mil., This solicitation is being issued under Solicitation Number W9124R-16-T-0041 which will result in a single-award, firm fixed price contract on a Brand Name Only basis with Carestream Health Inc. (Carestream NDT) located at located at 150 Verona Street Rochester, New York 14608-1733. This new acquisition acquired under this purchase must be form, fit, and function with existing equipment. The North American Industry Classification System (NAICS) code is 334517, Irradiation Apparatus Manufacturing, with a size standard of 1,000 Employees. The requirement is being solicited as a 100% small business set-aside. Quotes will be evaluated on a Lowest Price Technically Acceptable (LPTA) basis and tradeoffs are not permitted. Award will be made on an all or none basis. An offeror must quote on all items in this solicitation to be eligible for award. This item will be acquired using Federal Acquisition Regulation (FAR) Part 13 procedures., The United States Yuma Proving Ground, Yuma, Arizona is requesting quotes for Carestream Industrex HPX-1 Computed Radiography (CR) System, reference Attachment 2 - Specification of Work. This contract will consist of Contract Line Item Number (CLIN) 0001, Quantity: 1, Unit of Issue: Each, Description: Carestream Industrex HPX-1 Computed Radiography (CR) System., All quotes shall include transportation costs to USAYPG Yuma, Arizona 85365-9498 and be clearly marked with solicitation number W9124R-16-T-0041 and emailed to the Point of Contact (POC) listed below or sent by facsimile to 928-328-6534. Arizona vendors are to include Arizona Privilege Tax. Shipping and taxes (if applicable) shall be incorporated into your proposed price. Payment will be made through Wide Area Work Flow (WAWF). Delivery shall be made to USAYPG, AZ 85365-9498 with FOB Destination. All responding vendors must be actively registered with the System for Award Management (SAM) prior to award of contract.
