- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2016-94596 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a fixed-price contract for the following requirement. Please either fill in the dollar amounts, or provide a separate excel spreadsheet containing the same information including a list of contract line item number(s) (CLINs) and items, quantities, units of measure, and options (if applicable). Please see attached memorandum for additional details regarding CLINs and expected CLIN structure. All work shall be performed according to the attached Statement of Work (SOW). Your offer shall include the following: * total price for CLINs 0001-0004 * capabilities in accordance with the Statement of Work (SOW) and evaluation factors Delivery Date: as soon as possible, not to exceed twelve (12) months Delivery Address: CDC (Roybal Campus) 1600 Clifton Road, NE Building 23 Atlanta, GA 30329 Acceptance and FOB point of destination. The quote format is at the discretion of the Offeror. It is the Offeror's responsibility to be familiar with the applicable clauses and provisions. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-87-2 (6 April 2016). A fixed-price purchase order will be issued in writing to the successful Offeror.
