- Letter of Intent to Sole Source. Pursuant to the authority of FAR 13.106-1(b)(1)(i), the Contracting Officer may solicit from one source if the Contracting Officer determines that the circumstances of the contract action deem only one source is reasonably available. The Department of Health and Human Services (DHHS), Program Support Center (PSC), Acquisition Management Services (AMS), on behalf of the Office of the Assistant Secretary for Health (OASH), intends to issue a sole source to: Far Harbor LLC 816 Congress Avenue, Suite 1400 Austin, TX 78701 The HHS Office of Population Affairs (OPA) is developing clinical performance measures for contraceptive care, which will greatly improve the quality of contraceptive services and reduce rates of teen/unintended pregnancy and improve birth spacing. The measures will be uses by the Title X program's 4200 clinics that serve more than 4 million clients, as well as other public and private providers. OPA has developed the measures using administrative claims data; however, plans are already underway to start developing a version of the measures that uses data collected by electronic health records (such as history of sexual activity, and whether the client is seeking pregnancy). The development of an e-measure will greatly improve the measure because it will more precisely define the populations at risk of unintended pregnancy. The National Quality Forum (NQF), an independent non-partisan organization that sets standards and endorses measures that meet rigorous criteria related to the measures of public health importance, scientific validity, feasibility and usability, previously recommended an approach developed by Rand (Adams 2012). Far Harbor, Inc. statistical consultants conducted some preliminary work on the measures and adapted this methodological approach, to best suit the nested, hierarchical structure of claims data. NQF has reviewed and approved this methodological approach and is considering encouraging all future applicants to use this rather than the Rand approach. Additionally, Far Harbor used this approach to assess the reliability of four large programs; this involved analyzing a dataset of more than 800,000 clients from a non-profit national family planning program, and data from 3 state Medicaid programs, and has been granted exclusive rights to analyze the data owned by the national non-profit family planning program; this dataset can be used to test both the claims-based and electronic version of the measures. Ongoing statistical expertise is needed to complete the development of the performance measures for contraceptive care. Far Harbor's team of highly skilled consultants specialized in statistics, population research, survey design, data visualization, and scientific writing present them as uniquely qualified for to meet this need. Additionally, Far Harbor is solely responsible for the adaptation of the Rand methodology, and it is the only entity that has approved access to the data that was used to conduct the analyses included in OPA's application to NQF and is therefore the only organization that can respond with the speed needed. The NQF only considers perinatal measures every 2-3 years. Introducing new firms in the development of an electronic measure presents a risk of unnecessary delays and duplication of effort, further resulting in increased rates of teen/unintended pregnancy. Additionally, in the event of a delay due to additional analysis being requested, re-applying for NQF endorsement in 2-3 years would mean replicating much work that has already been conducted.
