- THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE; THE VA IS NOT SEEKING PRICING OR OFFERS. The Department of Veterans Affairs, VISN 20, in support of Veteran Affairs Puget Sound Health Care System is currently requesting information in order to determine the existence of viable commercial sources capable of providing an Advancement of the Patient Centered Medical Home (PCMH) Study. The intended contract period of performance is for one (1) year. Description of Services & Background: The patient-centered medical home (PCMH) is a primary care model characterized by several feature, including team-based care, enhanced access to care, and comprehensive care which encompasses preventive, acute, and chronic care. In April 2010, the Veteran Affairs Administration (VHA) launched the Patient Aligned Care Team (PACT), a national initiative (study) to implement a PCMH in primary care. The goal was to improve clinical continuity, coordination, and patient-centeredness. The PACT "teamlets" consists of a primary care provider (PCP), nurse care manager or registered nurse (RN), clinical associate and administrative clerk who work with the PCP to provide more comprehensive and proactive care. VISN 20 PACT Demonstration Lab Leadership Coordinating Center /Committee has and will continue to develop an enhanced PACT model to implement and evaluate the impact of a comprehensive medical home and the application of innovative tools. This is a non-personal service. A Contractor shall provide overall direction to VHA for implementation and evaluation of PACT for the Veteran Affairs Puget Sound Healthcare System. The Government shall not exercise any supervision or control over the contract service providers performing the service herein. Such contract service providers shall be accountable solely to the contractor, who in turn is responsible to the Government. The NAICS code assigned is: 541720, Research and Development in the Social Sciences and Humanities and the small business size standard is $20.5 million. The Government is not soliciting offers at this time, and it will be the Contracting Officer who will determine which suggested changes are accepted. This is a sources sought notice to gain knowledge of potential qualified sources. A solicitation is not currently available. Responses to this request will be considered for the purpose of determining how the requirements will be stated and how the procurement will be conducted. This sources sought announcement is not to be construed as a commitment by the Government, implied or otherwise, to issue a solicitation or to award a contract. No reimbursement will be made for any costs associated with providing information in response to this synopsis or any follow-up information requests.
