Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA248-16-Q-1036 is issued as a (Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83. Only emailed requests received directly from the requester are acceptable. Responses should be sent to Richard.frase@va.gov. No telephone responses will be accepted. To be receiving a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award. Offeror may register with CCR at www.orca.bpn.gov NAICS and Set-aside: The NAICS is 611430 and the small business size standard is $11.0 million. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. Description of Services: This statement of work is to: create an environment that facilitates the development of community among residents with dementia, their families, and their care providers in a dignified and respectful manner. Evidence based research has proven the effectiveness of these methods to improve cognitive gains, reduce aggressive behaviors, and improve overall quality of life. Additionally, to ensure staff members are educated in Montessori Inspired Lifestyle ®: Dementia Programming. This training will improve the healthcare environment for our veterans with dementia. This training is being pursued to address multiple action items that were cited during the inspection by The Long Term Care Institute) DELIVERABLES: -4 days of Montessori Inspired Lifestyle ®: Dementia Programming training. To be held on-site at the Lake City, FL Medical Center. -40 hours of follow up consultation toward implementation and sustainability of Montessori Inspired Lifestyle ® -Books for participants (one per trainee): "Montessori Inspired Lifestyle ® course workbook -Books for Lake City Medical Center Library (20 each): "A Different Visit - Paperback Edition "Hiding the Stranger in the Mirror -Books for Lake City Medical Center Library (10 each): "Montessori-Based Activities for Persons with Dementia, Volume 1 "Montessori-Based Activities for Persons with Dementia, Volume 2 "A Therapy Technique for Improving Memory: Spaced Retrieval -Activity Materials (10 each) "Montessori Inspired Lifestyle ® Activity Starter Kits - Early Stage "Montessori Inspired Lifestyle ® Activity Starter Kits - Moderate Stage QUALIFICATIONS/CERTIFICATIONS: "The contractor shall have the professional and technical experience to perform the work required by this statement of work. oThe instructor must have extensive training in Montessori Inspired Lifestyle ®: Dementia Programming "At least 10 years of instructing Montessori-based programming for persons with dementia "Instructor must possess a Ph.D. Cognitive Psychology / Gerontology "Must have provided instruction to at least 2 other VA facilities SCHEDULING AND HOURS OF WORK: "All work shall be scheduled with: Respective Nurse Managers "Hours of work. 7:30 am - 4:00 pm Eastern time Monday - Friday. "The 10 holidays observed by the Federal Government, are New Years Day, Birthday of Martin Luther King Jr., Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas or any other day specifically declared by the President of the United States to be a national holiday. EVAULATION AND SELECTION FACTOR: This service contract must be performed by an instructor(s) that have extensive training in Montessori Inspired Lifestyle ®: Dementia Programming. This training must have also been implemented in another VA hospital with positive results. Proposed Curriculum: (Day 1 Programming would be conducted to four separate groups over two days) Day 1: Four contact hours A.Who was Maria Montessori and why is her approach relevant? 1.Values - Respect; Dignity; Equality 2.Approach - Use of rehabilitation principles 3.Key Concepts - Promoting Independence; Reducing Challenging Behaviors B.Models of care for persons with dementia and their assumptions 1.Hospital 2.Hotel 3.Community C.Knowing the PERSON with dementia. 1.Persons with dementia are acting the way we would under the same circumstances 2.Know the individual if you wish to understand behavior and address challenges D.Strengths and abilities that remain in persons with dementia. 1.Persons with dementia have abilities that still can be accessed 2.Persons with dementia can and will learn new things. E.Addressing challenging behaviors using this approach. 1.Using strengths and abilities to create meaningful activity and social roles. 2.Engagement reduces challenging behaviors. F.Providing choice and empowering residents. 1.Retraining persons with dementia how to make choices. 2.Providing choice throughout the day in all of our interactions with residents. 3.A different view of the role of staff - enabling the resident. G.What are the benefits of adopting this approach? 1.Improved quality of life for residents and staff 2.Reduction in challenging behaviors 3.Improved relationships between staff and residents and among residents H.Next steps. 1.How the roll out will begin and unfold over time Day 1 Objectives: Participants will be able to: 1.Describe 3 key Montessori values as they apply to provision of care and services to residents 2.Describe how the Montessori method relates to rehabilitation principles, promotion of independence, and reduction of challenging behaviors 3.Compare the community model of care to more common models of care 4.Empathize more effectively with residents by understanding motivations behind residents' behaviors 5.Describe at least 4 preserved abilities in persons with dementia, including the capacity to learn new procedures 6.Describe how engaging persons with dementia effectively using their remaining capacities increases quality of life while reducing challenging behaviors 7.Understand the importance of providing choice to residents and describe at least 3 ways to provide choice in daily interactions 8.Describe at least 3 benefits for staff and residents by adopting the Montessori method 9.Understand next steps in the roll out of use of this method (Day 2: To be taken by staff designated as change "champions" who take part in Day 1 training and will be responsible for becoming peer trainers) Day 2: Six contact hours A.Review of Day 1 training: Key concepts 1.How to verify application of key concepts on units B.Assessing reading capacity in residents - Role playing and practice C.Creation of activity materials D.Individualized activities - Role playing and practice E.Small group activities - Role playing and practice F.Large group activities - Role playing and practice G.Increasing buy-in at all levels H.Using existing materials in new ways 1.Inventory of existing materials I.Resident committees and other ways to build communities J.Working with families K.Action Planning - Next steps Day 2 Objectives: Participants will be able to: 1.Assess correct application of key Montessori principles and values in the provision of care and activities to residents 2.Assess capacity of residents with dementia to read (at what print size) 3.Create materials for immediate use in activities 4.Implement Montessori-based individual, small group, and large group activities 5.Learn techniques for training other staff members in the implementation of the Montessori method 6.Describe at least 3 different methods for increasing buy-in from stakeholders at all levels within the organization 7.Apply Montessori principles to enable existing materials and activities to elicit better engagement in residents 8.Describe at least 4 different ways to better empower residents, including the creation and maintenance of resident committees 9.Describe at least 3 different ways to empower family members and other visitors so that they can better engage residents during visits 10.Develop an action plan to assess the process of roll out for adoption of the Montessori method Day 3: Hands-on work with Day 2 participants on units A.Peer trainers receive hand-on supervision and feedback from instructor B.Review of the day, what has been learned, and planning for the future Day 3 Objectives: Participants will be able to: 1.Monitor their own and others' ability to effectively implement the Montessori method in all interactions with residents 2.Further refine action plans for roll out of the Montessori method 3.Create a process for providing feedback to the instructor and staff at the Center for Applied Research in Dementia for ongoing support and consultation Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation.
