- STATEMENT OF WORK 1.Background: The contractor shall provide the labor, travel, necessary materials and supplies to perform the fabrication of the Dental Prosthesis for the Jesse Brown VA Medical Center Dental Service in Chicago, IL. Critical Requirements: The contractor must be within 50 miles of the facility so we can have a 48 hour turn-around for denture repairs and relines because eligible veterans need to be able to eat and not be without dentures. This is a standard of care. 2.Specific Tasks/Deliverables. Contractor shall fabricate dental prosthetics for the Jesse Brown VA Medical Center Dental Service. The chart below lists the required procedures and estimated annual quantities. Once these items are processed they shall be returned to the JBVA Medical Center Dental Service Laboratory in accordance with the turnaround times listed above. b.The following Quality Assurance Surveillance Plan is included: TaskIndicatorStandardAcceptable Quality LevelMethod of Surveillance QualityMarginal IntegrityProsthesis will demonstrate acceptable level of marginal integrity when evaluated on dental stone model or implant abutment. All picked ups and deliveries shall be performed these hours.
