- ACCESS CONTROL SYSTEM This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. (ii) The solicitation number is VA251-16-Q-0262 and is issued as a Request For Quotation (RFQ). DescriptionQuantityUnitPrice 1.The Contractor shall provide material and supervision to install Access Control System in accordance with the Statement of Work. 2.Installation1 1JB JB (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). (iv) There will be security clearance and installation on site at VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, Michigan 48105-2303. This solicitation is issued as a small business set aside. (v) The North American Industry Classification System (NAICS) code is 334290 with size 750 employees. NOTE: Offerors submitting quotations on Equal items shall provide the Manufacturer's Name, Brand and Part Number for each item providing a quotation on. (vi) To be technically acceptable the products quoted shall be Brand Name or Equal to Hirsch Readers. (vii) Installation and Delivery FOB Destination to VA Illiana Healthcare System, 1900 E. Main Street, Danville, Illinois 61832-5100. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, FAR 52.209-5DEV; 52.225-25; 52.216-1 (fill in: Firm Fix Price contract will be awarded), 52.232-2, VAAR 852-215-70, 852.233-70; 852.233-71 (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov. SUMMARY STATEMENT OF WORK STATEMENT I.SCOPE OF WORK: The Contractor will provide equipment and supplies (material) and labor (installation, integration, testing, and programming) for doors (including access readers, materials, and electrical networking) for the Physical Access Control System at VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, Michigan 48105. The contractor will install on site and obtain a security clearance from the VHA Ann Arbor Hospital. The contractor will integrate the newly added and installed access control system with the existing physical security system. The contractor will test all of the connections and the full operational system for completion of the job. III.PREVENTIVE MAINTENANCE: All preventive maintenance and physical repairs will be done by the contractor through a warranty. IV.TRAINING a. All contractor employees and subcontractor employees requiring access to VA information and VA information systems shall complete the following before being granted access to VA information and its systems: (1) Sign and acknowledge (either manually or electronically) understanding of and responsibilities for compliance with the Contractor Rules of Behavior, Appendix E relating to access to VA information and information systems; (2) Successfully complete the VA Cyber Security Awareness and Rules of Behavior training and annually complete required security training; (3) Successfully complete the appropriate VA privacy training and annually complete required privacy training; and (4) Successfully complete any additional cyber security or privacy training, as required for VA personnel with equivalent information system access [to be defined by the VA program official and provided to the contracting officer for inclusion in the solicitation document - e.g., any role-based information security training required in accordance with NIST Special Publication 800-16, Information Technology Security Training Requirements.] b. The contractor shall provide to the contracting officer and/or the COR a copy of the training certificates and certification of signing the Contractor Rules of Behavior for each applicable employee within 1 week of the initiation of the contract and annually thereafter, as required. c. Failure to complete the mandatory annual training and sign the Rules of Behavior annually, within the timeframe required, is grounds for suspension or termination of all physical or electronic access privileges and removal from work on the contract until such time as the training and documents are complete. V.REPORTING REQUIREMENTS: The Contractor shall be required to report to the Police Service Office at the VA Ann Arbor Healthcare System. Check-in mandatory. Contracting Officer Representative (COR) will be assigned after award, if required. VI.SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default.
