May 06, 2016 SFO # VA101-15-R-0082 West Haven, CT 1. It has come to our attention that there is a minor formula error in "Pt 08B(2)-Attachment 1 to 1364 Rev 04-13-16". The error is in Row 75 of the document. The formula totals for "Total Physical Security Lump Sum Cost:" in columns E through Q are sourcing from the wrong column. Attachment #1 to Form 1364A has been revised to reflect change. Please ensure your response includes the revised, updated form, "Pt 08B(2)-Attachment 1 to 1364 Rev 05-05-16" 2.Question: (Referencing Section 8.1 UTILITIES), Does this mean VA pays their own utilities? Answer: No. The cost of utilities is to be included as part of the rental consideration. PART 8 FORMS - The form listed below has been replaced with an updated version. Please ensure your response includes the revised, updated forms below: "Attachment #1 to GSA Form 1364A ("Proposal Summary") END OF AMENDMENT
