- Proper ID Samples Personal History Statement (PHS) Form 5532-53 (Rev 6-13) Form 5532-51 (Rev 6-13) COUNTRY LIST for Form 5532-53 (Rev 6-13) DRAFT REQUEST FOR PROPOSALS, N62649-16-R-0110, FOR REGION 3 -HUSBANDING SERVICES FOR THE PACIFIC FLEET (PACFLT) DRAFT REQUEST FOR PROPOSALS, N62649-16-R-0110, FOR HUSBANDING SERVICES FOR THE PACIFIC FLEET (PACFLT) This draft RFP is being issued in order to obtain feedback from potential offerors with regard to the terms and conditions outlined herein. Potential offerors are welcome to provide feedback with regard to any term or condition of the solicitation, but it is noted that the Government is specifically seeking feedback with regard to the following issues in order to determine appropriate requirements with regard to the Price Proposal preparation instructions in Section L: Please provide an answer for each ELIN item/service, including grayed out and "Reserved" service/supply. 1. For all priced ELINs in Exhibit AA-AU, what services are available in each port? Please enter your answer (Yes/No) in column U of Exhibit AA-AU. 2. With regard to Exhibit AA-AU, which services/items can or cannot obtain pricing for during the solicitation phase? Please enter your answer (Yes/No) in column V of Exhibit AA-AU. 3. With regard to Exhibit AA-AU, what are the port tariff items in each port? Please enter your answer in column W (Yes/No) of Exhibit AA-AU. 4. With regard to Exhibit AA-AU, are you able to obtain pricing for the services that you have identified as port tariff? Please enter your answer in column X (Yes/No) of Exhibit AA-AU. Currently, Section L is drafted such that offerors would be required to propose unit prices for ALL ELINS IN EXHIBITS AA-AU, WHICH ARE NOT GRAYED OUT. Feedback regarding the issues outlined above is being requested from industry in order to modify this to suit the marketplace. Please note "Schedule Notes" and PWS might state some items/services are Reserved (will be priced at task order level), however if industry can provide pricing for this reserved item/service on the contract level, please provide feedback stating it can be priced at the contract level. Questions from industry are welcome at any time during this draft RFP. Industry Day: It is noted that the Government intends to conduct an industry day while this draft RFP is being advertised. An industry day is tentatively scheduled for 06 June 2016 at Navy Supply Systems Command (NAVSUP) Fleet Logistics Center Yokosuka (FLCY) located at Yokosuka, Japan. One-on-one sessions will also be conducted with vendors, if requested, in order to permit detailed discussions regarding issues that may be considered business sensitive. The requests for one-on-one sessions must be requested no later than 13 May 2016, to include required paperwork complete with attendees' information. Offerors that plan to attend must communicate by e-mail the names, email addresses, and nationalities of each participant to the following points of contact no later than 13 May 2016 in order to coordinate for base access. In addition, required paparwork must be completed and submitted by each attendee in accordance with the instructions below. It is requested that vendors limit attending personnel to two (2) persons per vendor. ALL DOCUMENTS MUST BE SUBMITTED BY 13 MAY 2016 NO LATER THAN 11:59PM (local time in Japan). 1. Each individual attending the industry day must COMPLETE AND SIGN attached form called "Personal History Statement (PHS)". Please sign and date in Block 33 ONLY. 2. Refer to attached excel sheet called "Country List for Form 5532-53 (Rev 6-13)." If the individual attending the industry day is from one of the listed countries, then the individual must fill out the GUEST'S INFORMATION section of Form 5532-53 (Rev 6-13), and provide a copy of his/her passport. Please be advised, no vehicles can come on base. P.O. Box addresses are not accepted on the form. 3. Refer to attached excel sheet called "Country List for Form 5532-53 (Rev 6-13)." If the individual attending the industry day is NOT from one of the listed countries, then the individual must fill out the VISITOR DATA section of Form 5532-51 (Rev 6-13). Please be advised, no vehicles can come on base. P.O. Box addresses are not accepted on the form. *** Each individual must bring proper identification when they enter the base. Samples of proper identification for Yokosuka Base is attached for reference. The U.S. Governemnt will not make any exceptions and will not be responsible for individuals who do not have proper identification. *** *** All travel arrangements, including passports and visas, lodging, transportation and meals will be the responsibility of the vendor. ***
