- ATTCH 4: CLIENT AUTHORIZATION ATTCH 3: PAST PERFORMANCE QUESTIONNAIRE ATTCH 2: DEPT. OF LABOR WAGE DETERMINATION ATTCH 1: QASP RFP HSFE40-16-R-0002 This is a solicitation for a commercial service prepared in accordance with Federal Acquisition Regulation (FAR) Subpart 12 in conjunction with Subpart 15, and with the format in Subpart 15.204, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and proposals are being requested. The Solicitation number is HSFE40-16-R-0002 and is issued as a Request for Proposals (RFP). This solicitation document incorporates all mandatory commercial item provisions and clauses that are in effect through Federal Acquisition Circular (FAC) FAC 2005-87-2 / 04-06-2016 The North American Industrial Classification System (NAICS) code is 541611, and the small business size standard is $15.0 million. This is a competitive 8(a) small business set-aside. Only e-mailed requests for additional information will be accepted at Carmen.Ragin@fema.dhs.gov through 12:00 PM Eastern Standard Time (EST), May 16, 2016. Offerors are invited to submit their proposals in response to this notice by 12:00 PM (EST), June 6, 2016. Proposals submitted in response to this notice shall include the solicitation number in the subject line and be signed, dated and submitted via email to Carmen.Ragin@fema.dhs.gov.
