- Sources Sought Notice - VA118-16-C-0059 Addition of Alaska Fee Introduction: This is notice Sources Sought Notice to modify/update the Fee Basis Claims System (FBCS) software owned by Document Storage Systems Inc. to allow for the automatic calculation of payments of claims in the state of Alaska for anesthesia and professional services provided by non-VA providers in Alaska. The Alaska VA Health care system (AVAHS) provides healthcare services to approximately 21,000 eligible veterans living in the State of Alaska. The AVAHS operates a large multi-specialty outpatient clinic in Anchorage, and small community based clinics in Fairbanks, Kenai, and the Matanuska-Susitna Borough. The AVAHS also operates a VA Outreach Clinic in Juneau, Alaska and another VA Outreach Clinic in the community of Homer, Alaska. The VA spends approximately $107 million per year on purchased care for Veterans in Alasks. VA facilities in the rest of the United States reimburse physicians and other healthcare providers and facilities according to the Center for Medicare & Medicaid Service (CMS) fee schedules. With the exception of physician services, the CMS fee schedules are adequate to allow for sufficient access to purchased care for Veterans in Alaska. However, the CMS physician fee schedule, particularly for specialty care, is considerably lower than commercial rates and other Federal payers in Alaska. Since a large portion of VA health care provided in the State of Alaska is obtained from non-VA sources, the determination was made that a special payment circumstance exists. Therefore, to ensure that amounts paid to health care providers is equitable, while practicing cost containment, VA established the Alaska Fee Schedule; an Alaska-specific payment schedule for inpatient and outpatient non-VA physician and other provider services. These non-VA providers are paid utilizing the Alaska Fee Schedule in accordance with 38 CFR 17.56(b).
