Description

1.This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number VA262-16-Q-0794 is being issued as a Request for Quotation. It is the offeror's responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-87, effective April 6, 2016. 2.The North American Industry Classification System (NAICS) code for this acquisition is 541620 - Environmental Consulting Services and the small business size standard is $15 million. This RFQ is being solicited as 100% small business set-aside. Offerors must be registered in the System Award Management (SAM) at https://www.sam.gov, in order to be considered for award. 3.Offeror are requested to provide pricing for all three (3) contract line items (CLIN) as specified in Attachment 1 - Price Schedule of Supplies/Services of this RFQ. The following documents are also attached for your reference to assist in completing pricing form, Attachment 1 - Price Schedule of Supplies and Services: 3.1Attachment 1-1: Rule 2202 Registration Form 2016 3.2Attachment 1-2: Rule 2202 - 2014 Annual Program Compliance Forms 3.3Attachment 1-3: Rule 2202 - 2013 Annual Program Compliance Forms 4.VA Loma Linda Healthcare System, 11201 Benton St., Loma Linda, CA 92354, has a requirement for emission credit transfer, filing and establishing and maintaining rideshare matching database to ensure full compliance with South Coast Air Quality Management District (SCAQMD) Rule 2202 - On Road Motor Vehicle Mitigation. In accordance with (IAW) the Statement of Work (SOW) described in Attachment 2 - SOW, the Contractor shall provide all resources necessary to accomplish all requirements as specified to ensure proper and effective emission reduction program are in place and in compliance with applicable laws and regulation for air pollution and emission controls. 5.The period of performance shall begin June 1, 2016 through May 31, 2017. The Contractor shall provide all required documentations to meet the compliance due date for the plan and fees on or before June 10, 2016. 6.Provisions and clauses FAR 52.212-1 Instructions to Offerors - Commercial Items, 52.212-4 Contract Terms and Conditions- Commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items are applicable to this to this acquisition. Full text of provisions and clauses may be accessed electronically at http://farsite.hill.af.mil. 7.The Government intends to award a single, firm-fixed price purchase order resulting from this solicitation whose offer represents the lowest price, technically acceptable offer that is determined to be complete, accurate, responsive, and conforms to the solicitation requirements; and whose offer is judged by an overall assessment of the evaluation criteria and considerations specified in this solicitation. The Government will evaluate price quote for fairness and price reasonableness. This solicitation will be awarded to the lowest price quote after the quoter's technical capabilities has been determined to be acceptable for all non-price factor. The quoter shall provide technical capability statement demonstrating at least a minimum three (3) years of experience and technical knowledge in providing emission credit transfer including all requirements specified in Attachment 2 - SOW. 8.Offeror(s) must submit a completed copy of FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items or a copy of offeror's annual offeror representations and certifications (ORCA) document from www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items. Offeror(s) shall submit their offer in response to this RFQ as follows: 8.1All submitted documents must include company name, point of contact, address, telephone number, email address, DUNS number and RFQ no.VA262-16-Q-0794 8.2Completed pricing form - Attachment 1 - Price Schedule of Supplies/Services 8.3Technical offer as required in Section 7 of this RFQ 8.4Completed section 8 of Attachment 2 - SOW 9.Offers are due on May 23, 2016, 12:00 PM, Pacific Daylight Time (PDT).
