W91YTZ-16-T-0214 Solicitation W91YTZ-16-T-0214 Solicitation Information The Womack Army Medical Center-Contracting Cell Regional Health Contracting Office - Atlantic (Provisional) Army Health Contracting Activity (Provisional) US Army Medical Command (MEDCOM), Fort Bragg, NC 28310, intends to solicit, negotiate, and award a contract for Pre-filled Syringes for the Department of Anesthesiology Service. **** All responses must be received by 23 May, 2016, 8:00 a.m. EST. Email responses to mary.a.mitchellmartin.civ@mail.mil ***** This requirement is Unrestricted. Proposals are being requested and this solicitation is expected to result in the award of a firm-fixed price contract. The resulting award will be made on SF 1449, for commercial supplies. This requirement is for five (5) shipments (1 June 2016 - 30 Sep 2016). There will be a total of five (5) shipments; one (1) each month for June, July, August delivered on the 1st day of the month and two (2) shipments for September with one (1) being on the 1st and the second being on the 29th. The following must be addressed in your technical proposal or it will be rated Unacceptable: THE PREFILLED SYRINGES MUST BE COMPOUNDED AS A 503B FACILITY THAT IS INSPECTED BY THE FDA. ADDITIONALLY, THE REQUESTED SYRINGES MUST COMPLY WITH ASTM D4774-11e1 (American Society for Testing and Materials) STANDARDS FOR LABELING ANESTHESIOLOGY DRUGS. *** A Sanitized (No names, company names, prices, phone numbers or addresses) copy of the Technical proposal shall be submitted in response to the solicitation. Clauses and provisions throughout this solicitation can be viewed by accessing website www.farsight.hill.af.mil and https://acquisiton.gov/far. FAR provision 52.212-1 [Instructions to Offerors Commercial Items] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the System for Award Management (SAM) program. The SAM can be accessed via the internet at www.sam.gov. Confirmation of SAM registration will be validated prior to awarding a contract. "CAUTION, READ" -- FAR provision 52.212-1 and the CLINS 0001 through CLIN 0010 -- "IN DETAIL" before submitting your proposal. You must submit pricing separately from your technical proposal. Fill in prices on each CLIN line; the Unit Price is the cost of one shipment of the pre-filled syringes requested for that line. For example on CLIN line 0001 your unit price would be the cost of 25 each ATROPINE, 0.4MG/ML, 3ML SYRINGES (which equals one shipment). Your Total Amount would be the cost of 5 of these shipments. You must submit past performance that shows your experience in delivering supplies of this type. "Offeror is required to submit information on a single page identifying federal, state, local Government and private contracts performed within the past three years (from the date this solicitation is issued) which are similar in scope and complexity to the effort described in this solicitation. " ELECTRONIC OFFERS. Offerors may submit electronic offers in response to this solicitation. The offer must arrive by the time specified in the solicitation to the following email address: mary.a.mitchellmartin.civ@mail.mil. The Government will not be responsible for any failure of transmission or receipt of the offer, or any failure of the offeror to verify receipt of the emailed offer. The Government intends to award a firm-fixed price contract. Offerors must quote a firm-fixed price for each Contract Line Item. Any questions with respect to the solicitation shall be submitted in writing to Ms. Martin at mary.a.mitchellmartin.civ@mail.mil by 1:00 p.m. EST 19 May, 2016. Telephone calls will not be accepted nor responded to. Responses will be emailed to all interested offerors. The technical information required for evaluation purposes, shall not exceed 10 pages. PROCESS FOR AWARD. Award will be made on a lowest price, technically acceptable basis. Discussions may be conducted whenever the Contracting Officer deems it appropriate. The contract will be awarded strictly in accordance with the criteria and process established in FAR 12.
