Description

Wage Determination Pricing Schedule Provisions and Clauses Statement of Work Preventative Maintenance; non-personal services for the Hopi Health Care Center (HHCC), Polacca, Arizona. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. Provisions and Clauses in effect are as issued in the most current OMB Circular (2005-87). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. NAICs Code is 811310, Size Standard is $ 7,500,000.00. The requirements for the solicitation include submitting a Technical Proposal; providing references as outlined in the provisions and clauses; and filling in a Pricing Schedule for the services contemplated in the Statement of Work and the offerors' proposal.
