- Notice of Intent to Sole Source The Naval Medical Center San Diego (NMCSD) intends to award a sole source purchase order under the authority of FAR 13.106-1(b) (1) (i), due to only one source being available to meet the Government's needs, to Musculoskeletal Transplant Foundation Inc. (MTF) 125 May St Ste 300 Edison, NJ, 08837-3264 The requirement is for ACF allograft spacers to perform Neurological surgery. The following items are needed: Item # Description Qty 001 DBX Dental 1cc 30 002 DBX Putty 5cc 30 003 Illium Tricortical Strip 1 004 ACF Spacer Lordotic 10mm 2 005 ACF Spacer Lordotic 7mm 6 006 ACF Parallel 10mm 1 007 Cancellous chips 5cc 30 008 Cancellous chips 15cc 30 -The Allograft spacers must 1) be compatible with Stryker instrumentation currently owned by US Naval Hospital Okinawa. 2) have pyramid surface striations to provide for maximum surface area. 3) provide the option for packing with autograft blood and bone fragments. 4) be available in both parallel and lordotic. -DBX Putty must 1) provide a moldable consistency. 2) osteoconductive and osteoinductive and has documented to be Non-allergenic. 3) has a reliable PH of 7.2, similar to human blood. -Cancellous must 1) have good osteoconductive properties. 2) well suit to fill bone structure defects. MTF is the only known company that can meet the requested requirement of providing the Allograft spacers, DBX Putty and Cancellous chips due to MTF has an allograft inter body fusion spacer that is compatible with currently owned instrumentation, has pyramid surface striations, has a packing option, and is available in the needed size range and configuration. Delivery is required on the following date: May 31, 2016 This notice of intent is not a request for competitive proposals and no solicitation document exists for the requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to perform the required work. Capability statements shall not exceed 7 (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order. Capability statements are due by 10:00 AM Local Time, 23 May, 2016.
