Description
This is a sources sought to determine the availability of potential sources having the capabilities to provide the SPECIFIED REQUIREMENT. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. Questions should be submitted by email to michael.giffon@va.gov. Provide only the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 325412, Pharmaceutical preparation manufacturing. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. SPECIFIED REQUIREMENT: Network Contracting Office 10, Marion, IN is seeking sources for a potential contractor to supply the following items for the ILLIANA VETERANS AFFAIRS MEDICAL CENTER IN DANVILLE, IL: ITEM NO. DESCRIPTION QUANTITY UNIT See statement of work and estimated quantities below. STATEMENT OF WORK Nuclear Medicine Radiopharmaceuticals for VA Illiana Healthcare System, Danville, IL, Imaging Service 1. General Description: VA Illiana Healthcare System (VAIHCS) has a requirement for radiopharmaceuticals for the nuclear medicine department. Radiopharmaceutical preparations of doses, tagging of blood, testing performed at contractor facility provided by a board-certified radiopharmacist and delivered to the Nuclear Medicine Department at Illiana VA Health Care System at 1900 E. Main St. Danville, IL 61832. Deliveries will be made Monday through Friday from 6:00 am to 3:00 pm. Contractor shall provide emergency deliveries as requested by the VA Nuclear Medicine Departments. Personnel assigned by the contractor to perform services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Because of the critical need of these supplies in a timely manner, only vendors able to meet the 2 hour delivery time the Illiana VA Medical Center outlined in paragraph 6 need to respond to this RFQ. 2. Clinical Duties: Contractor's employees will give deliveries directly to the Nuclear Medicine Technologist on duty. Delivery personnel will be bonded by the contractor. 3. Administration: a. Contractor will bill on a monthly basis. b. All services rendered will be itemized by date of service, radiopharmaceutical provided, and unit cost. 4. Security and Confidentiality: a. Responsible for maintaining Automated Data Processing (ADP) security. b. Responsible for maintaining patient information in the strictest confidence in accordance with the Privacy Act. c. The contractor must provide evidence, in the form of a certificate, to show that they actively procure Tc-99m from facilities that produce non-HEU (High Enriched Uranium) radioisotopes. All Tc-99m shall be procured from non-High Enriched Uranium (HEU) companies. 5. Work Hours: a. The services covered by this contract shall be furnished by the contractor as defined herein. The contractor shall not be required, except in case of emergency, to furnish such services on a national holiday or during non-working hours as described below. b. Work hours: Monday through Friday, 7:00 am - 4:30 pm. 6. Delivery Orders: All routine orders for Radiopharmaceutical doses will be placed to the contractor one day before they are needed and prior to the clinic close of business via phone or internet. The order will include specified activities of doses and calibration times. The contractor will deliver the first run of ordered Radiopharmaceutical doses to the Nuclear Medicine Department before 0700 (CST) on the day specified to ensure proper inventory and receipt of the shipment is done by qualified personnel. A subsequent run of ordered radiopharmaceuticals will arrive by 1130 (CST) and if necessary a 3rd run will arrive for all add-on procedures ordered prior to the close of business. All emergency after hour orders of radiopharmaceutical doses will be placed by an on call Imaging staff Technologist on an emergency need basis. The contractor will deliver all after hour orders of Radiopharmaceuticals within two hours of the Radiopharmaceutical doses being ordered to the Nuclear Medicine Department. Due to the demands of the Medical Center care requirements, there shall be no restriction to the number of orders which may be placed by VA, nor the number of deliveries required per day. In the event that the requirements may be determined in advance, a longer delivery time will be scheduled accordingly. The contractor shall provide a daily packing slip accompanying all Radiopharmaceutical shipments to the Nuclear Medicine Department for all orders placed. The packing slip shall include a bar code that works with NMIS for tracking purposes. The delivery order shall specify: contract item (s) and quantity, time (s) of delivery and applicable VA order number. The contractor will calibrate all Radiopharmaceutical doses to ensure proper activity ranges of +/- 10 percent of the specified ordered dose based on specific requirements provided by Nuclear Medicine Department based on guidelines outlined in 10 CFR Part 35. The contractor will ensure that all Radiopharmaceutical doses provided are compounded and calibrated by or under the supervision of a certified nuclear pharmacist. The contractor shall provide courier service that is in accordance with Department of Transportation, Nuclear Regulatory Commission and other applicable regulating authorities enabling the delivery of all Radiopharmaceuticals. All couriers shall be licensed and shall submit all required paperwork in order to be granted access to VAICHS. The contractor shall be responsible for the removal of all used and unused radiopharmaceutical doses from the clinic within 72 hours of delivery. The contractor shall provide any training or education required to meet commercial standards related to use of radiopharmaceuticals to all staff Nuclear Medicine Technologists stationed at VAIHCS. The contractor shall be able to work within NMIS, the current Nuclear Medicine management program at VAIHCS. This shall allow for the receipt, inventory and proper documentation of Radiopharmaceutical doses dispensed. 7. Delivery Location: Illiana VA Health Care System Nuclear Medical Center 1900 E. Main St. Danville, IL 61832 "Deliveries will be made Monday-Friday (excluding holidays which are listed below) between the hours of 7:00 AM and 4:30 PM, except in the case of emergency. Federal Holidays are as follows and include any other days specifically declared by the President Of the United States to be deemed a holiday. "Deliveries will be supervised by the COTR or their designee before beginning delivery operations. Contractor will contact the COTR prior to delivery to arrange for delivery supervision. 8. Delivery Commitment: Time of delivery specified or mutually agreed to at the time of receipt of telephone order shall become mandatory upon the Contractor. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract in accordance with the provisions for default. The contractor shall notify VAIHCS Nuclear Medicine Department immediately should the inability arise to provide Radiopharmaceutical doses. 9. Person Authorized to Place Orders: In addition to the Contracting Officer, the COTR is authorized to place orders under this contract, as well as any Certified Nuclear Medicine Technologist employed by the Illiana VA Health Care System VA Hospital. 10. The services specified herein may be changed by written modification. The modification will be prepared by the VA Contracting Officer prior to becoming effective, shall be approved by the VA Undersecretary for Health or his/her designee. 11. Period of Performance: 1 June 2016 - 31 May 2017. 12.
