- RFP Attachment 4 - Sample Task Order Attachment 3 - Resume Template Attachment 2 - Conflict of Interest Declaration Matrix Attachment 1 - Pricing Template SF-33 THIS OPPORTUNITY IS AVAILABLE ONLY TO CONTRACTORS UNDER 100% SMALL BUSINESS (SB) Set-Aside. This announcement constitutes the Synopsis/Solicitation notice; the written solicitation is attached and quotes are being requested. Solicitation number DTFT6016R00004 is being issued as a REQUEST FOR PROPOSAL (rfp). The solicitation document and incorporated provisions and clauses are those in effetct through Federal Acquisition Circular (FAC) 2005-74 effective 1 July 2014.. All applicable provisions and clauses are included in the actual solicitation. The Department of Transportation, Federal Transit Administration, 1200 New Jersey Avenue SE, Washington DC 20590 is seeking small business concerns to submit proposals for the requirement of Title VI, Equal Opportunity and Disadvantaged Business Enterprises Compliance reviews, solicitation numbered DTFT6014R00004. The anticipated closing date for this requirement is June 15, 2016. North America Industry Classification System(NAICS) code is 541611, and the small business size standard is $14M. The solicitation is for 100% Small Business Set-Aside acquisition. The Government intends to issue more than five (5) Indefinite Delivery Indefinite Quantity (IDIQ) contracts for conducting compliance reviews on FTA gRANTEES Title VI, EEO & DBE programs. Quotations must be good for 30 days after the RFP closing date. Questions concerning this solicitation must be received by the Government via email no later than 12:00 PM Eastern Standard Time (EST) June 15, 2016. Any quote that is submitted by a vendor that is not a small business will not be considered for award.
