Description

Pursuant to FAR subpart 13.106-1(b)(1), the U.S. Air Force Academy 10th Contracting Squadron, USAF Academy, CO intends to issue a sole source blanket purchase agreement for a period of one year to the Joint Restoration Foundation (JRFOrtho) Inc. to purchase tissue grafts for USAFA Clinic, USAF Academy, CO 80840-2315. This notice is not a request for proposal. Any firm that believes it can provide the stated supplies can provide supporting documents demonstrating their product capability and specifications. Responses must be submitted via email to include the company's name, point of contact, address, phone number, DUNS number, Tax ID, business size, CAGE code and solicitation number. No telephone or facsimile requests will be accepted. Information can be sent to JOhn Keith at John.keith.4@us.af.mil. All responses must be received by 4:00 MST 3 June 2016. This procurement will be prepared in accordance with FAR Part 13, using Simplified Acquisition Procedures.
