Note: This notice is published in accordance with Federal Acquisition Part (FAR) Part 10, Market Research and FAR Part 15, Exchanges with Industry before Receipt of Proposal. This notice serves as a Sources Sought in support of market research and/or planning purposes only and does not constitute a solicitation. This notice is not a confirmation that the Government will contract for the items or services contained in the sources sought, nor is any commitment on the part of the Government to award a contract implied. This notice does NOT constitute an Invitation to Bid, a Request for Proposal, or a Request for Quote. The Government shall not pay and/or reimburse respondents and/or associates for any costs incurred to submit the requested information. All information received in response to this sources sought that is marked Proprietary will be handled accordingly. In accordance with FAR 15.201(c), response to this sources sought is not offers and cannot be accepted by the Government to form a binding contract. STATEMENT OF WORK SCOPE: VAPHS is required to have all dampers inside of duct work identified and tested every six years. VAPHS requires a Contractor to conduct field verification of all damper locations, testing of all dampers, and repair of any failed dampers. The Contractor agrees to provide the following damper locations and subsequent inspection services with respect to VAPHS's dampers at the University Drive (UD) and Heinz (HZ) Campuses. University Drive Campus University Drive C Pittsburgh, PA 15240 Heinz Campus 1010 Delafield Road Pittsburgh, PA 15215 TASKS: Conduct field verification of all dampers at VAPHS University Drive and Heinz campuses to include the following tasks. 1.Use commercially reasonable efforts to locate, operate (i.e., actuate) all dampers in the University Drive campus. Identify inoperable dampers. Location and testing in accordance with National Fire Protection Association Code (NFPA (http://www.nfpa.org/). 2.Remove and reset fusible links on Fire Dampers, to verify such Fire/Smoke Damper fully closes and if necessary, replace any fusible link that is compromised as required per NFPA (http://www.nfpa.org/. 3.Lubricate all moving parts on each damper 4.Clear each damper of any debris that would impede such damper's normal operation 5.Coordinate with Contracting Officer Representative (COR) and Safety to place VAPHS's fire alarm system in "test mode" if necessary to conduct inspection of the smoke and/or combination dampers. 6.Manually activate actuators on electric and pneumatic smoke and/or combination dampers to verify proper operation of such damper. 7.Defects or non-functioning dampers or fusible links are to be recorded and reported in the final report as defined in this contract. If the damper is inaccessible, a visual inspection must be made and recorded and reported immediately to COR. 8.If there is no existing access to a damper, then proper size access doors, as reasonably determined by Fire and Smoke Damper Inspection Contractor and agreed upon with COR will be installed to facilitate inspection. 9.Identify on VAPHS drawings the location of each damper (update all electronic files in the AutoCAD file to reflect current field conditions at VAPHS), along with placement by contractor of identification stickers on ceilings or other location to assist in locating such dampers following inspection. 10.Installation of Access Doors when necessary 11.Installation of Actuator, when required. 12.Installation of Dampers when required. 13.Damper repairs. 14.Damper replacement. DELIVERABLES: On conclusion of the performance of the field verification and inspection services, Contractor will provide to COR a report (in electronic spreadsheet) and updated AutoCAD drawings with all damper locations which is intended to provide VAPHS evidence of all damper locations, damper inspection for submission to applicable agencies, such as The Joint Commission and VAPHS's local fire protection agencies. Report shall be in electronic spreadsheet format and a digital copy given to COR. The Report shall include the following and must be submitted within 30 days after date of award on contract. All installations of access doors, installation of Actuators, damper repairs, damper installations, and damper replacements must be completed 30 days after report is delivered (60 days after date of award). 1.Location of dampers (building number, floor number, room number, air conveyance system) - 2.Type of damper, (fire damper, smoke damper and combination fire and smoke damper) 3.Inspectors name 4.Damper identification number 5.Test results (pass or fail). 6.With failure or inaccessibility, a detailed written comment will be provided for each occurrence. 7.Digital photographs of each damper before and after performance of the inspection services (showing each damper opened and, where operable, closed). 8.Updated AutoCAD drawings reflecting all damper locations CONTRACT REQUIREMENTS: Contractor must adhere to the following requirements 1.Damper Accessibility a.If the only access point is longer than an arm's reach away from the damper, perform a visual inspection to the limit allowable. Look at the frame for signs of kinking or twisting. Look for rust or build-up of any kind that may keep the damper from closing. Look for the obvious, such as wire or conduit running through the damper. Record your observations and have them available for review by COR. b.If the damper is completely occluded by layers of utilities and is totally inaccessible, record that fact and notify COR. c. If a damper is inaccessible, the damper will still be located, mapped on VAPH's drawing, labeled at its physical location, and included in the Report. The damper will be included in the final damper count and reflected on the final invoice. 2.Required Hours of Work: All Work to be performed during hours of 4:00p.m. To 12 midnight so as to not disrupt hospital operations. All systems will be back in operation by the end of the shift. Work areas will be returned to as found conditions. Schedule for location and testing to be submitted and approved by COR prior to commencement of work. 3.Contractor shall schedule maintenance and inspections in collaboration with COR. Contractor to coordinate access to all HVAC mechanical spaces and equipment and shut down of equipment systems with the COR. 4.Contractor shall adhere to Medical Center Requirements outlined in attachment A of the solicitation. 5.The contractor shall be fully responsible for compliance with all VA, local, state, and Federal environmental/occupational safety laws, rules and regulations. The contractor shall follow life and safety codes, and take necessary actions to avoid conditions that may be hazardous to the health and safety of hospital personnel and patients. The contractor is responsible for meeting all applicable OSHA and VA guidelines for safe working processes (http://www.cfm.va.gov/til). 6.Infection Control Procedure. Contractor will perform the inspection services in a workmanlike manner. While performing such inspection services, Contractor will adhere to VAPHS's infection control risk assessments ("ICRAs"), Policy EC-051, Attachment D. No work can commence without signed ICRA permit from Contractor, COR, and Infection Control. "Contractor responsible for conducting a pre-work risk assessment for the transmission of Tuberculosis (TB) to the contracted workers based upon the contract required locations throughout the medical center, patient population, hospital layout, and the defined risk as outlined in the "CDC Guidelines for preventing the transmission of Mycobacterium Tuberculosis in Health-Care Setting, 2005" (see subpar. 5v). "Contractor to review of all contract worker Tuberculosis (TB) screening requirements and options (reference VHA Directive 2011-036 Safety and Health during Construction). The contractor shall also be notified that anyone failing to meet these requirements is not permitted to work onsite and/or may be removed at any time without notice until these requirements are met. 7.Period of Performance: Contractor shall complete all location, testing, inspection and maintenance identified in this contract within 30 days of award of contract.
