Description
Statement of Work (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The solicitation number for this effort is AG-6395-S-16-0122. This solicitation is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2005-88, effective May 16, 2016. The Federal Acquisition Regulation (FAR) can be accessed on the internet at http://farsite.hill.af.mil/ (iv) This is a total small business set-aside. The appropriate NAICS code is 339999 with a small business size standard of 500. (v) Contract line item number(s) and items, quantities and units of measure: 0001 Yellow Sticky Traps Qty: 70,000 Unit Price: $______ Each (vi) Description of requirements of items to be acquired: See attached Statement of Work. (vii) Date(s) and place(s) of delivery and acceptance at FOB point: 6 weeks after award. FOB Destination to USDA APHIS PPQ Facilities and Operations, Building 6407, Moore Air Base, 22675 N. Moorefield Road, Edinburg, TX 78541. (viii) The provision 52.212-1, Instructions to Offerors - Commercial, applies with the following addendum:(b) Submission of quotations, is revised to read as follows: QUOTES MUST INCLUDE THE FOLLOWING: 1. Cover page referencing the solicitation number, company name, address, point of contact, and DUNS number. 2. Quoted Unit Price (Unit price must include delivery costs, FOB destination.) 3. Product information sufficient for the Government to determine that the product will meet the requirements. (Please note clause 452.211-70 Brand Name or Equal below.) 4. Acknowledgement of Amendments (if applicable) Submit quotes to Lisa Draves at lisa.a.draves@aphis.usda.gov (ix) Quotations will be evaluated in accordance with FAR Subpart 13.106-2, Evaluation of Quotations or Offers. Award will be made to the responsible offeror whose quote conforming to the solicitation is the lowest priced technically acceptable quotation. (x) The provision 52.212-3, Offeror Representations and Certifications - Commercial Items applies. Quoters shall complete online representations and certifications in the System for Awards Management (SAM) at www.sam.gov prior to specified response date/time. (xi) The clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies with the following addendum: (g) Invoices, is revised to include the following statement: All invoices shall be submitted through the Invoice Processing Platform (IPP) at www.ipp.gov. IPP is a secure, web‐based service that manages government invoicing. (xii) Clause 52.212-5, Contract Terms and Conditions Required To Implement Statues or Executive Orders - Commercial Items, applies to the acquisition. Also,the full text of a clause or provision may be accessed electronically at this/these address(es): http://farsite.hill.af.mil/ 52.232-40 Providing Accelerated Payments to Small Business Subcontractors 452.211-70 Brand Name or Equal (Nov 1996) (a) If items called for by this solicitation have been identified by a "brand name or equal" description, such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of products that will be satisfactory. Offers of "equal" products (including products of the brand name manufacturer other than the one described by brand name) will be considered for award if such products are clearly identified in the offer (see clause 452.211-71) and are determined by the Contracting Officer to meet fully the salient characteristics requirements listed in the solicitation. (b) Unless the offeror clearly indicates in its offer that it is offering an "equal" product, the offeror shall be considered as offering the brand name product(s) referenced in the solicitation. (c)(1) If the offeror proposes to furnish an "equal" product or products, the brand name(s), if any, and any other required information about the product(s) to be furnished shall be inserted in the space provided in the solicitation. The evaluation of offers and the determination as to the equality of the product(s) offered shall be the responsibility of the Government and will be based on information furnished by the offeror or identified in its offer as well as other information reasonably available to the contracting activity. Caution to offerors: The contracting activity is not responsible for locating or securing any information which is not identified in the offer and is not reasonably available to the contracting activity. Accordingly, to assure that sufficient information is available, the offeror must furnish as a part of its offer all descriptive material (such as cuts, illustrations, drawings, or other information) necessary for the contracting activity to (i) determine whether the product offered meets the salient charac-teristics requirement of the solicitation, and (ii) establish exactly what the offeror proposes to furnish and what the Government would be binding itself to purchase by making an award. The information furnished may include specific reference to information previously furnished or to information otherwise available to the contracting activity. (2) If an offeror proposes to modify a product so as to make it conform to the requirements of the solicitation, the offer shall include (i) a clear description of such proposed modifications and (ii) clearly marked descriptive material to show the proposed modifications. (End of Provision) 452.211-71 Equal Products Offered (Nov 1996) (a) Offerors proposing to furnish an "equal" product, in accordance with the "Brand Name or Equal" provision of this solicitation, shall provide the following information for each offered "equal" product: Brand Name or Equal Product identified by the Government in this solicitation:_ Offered Product Name:___________________________________________ Catalog Description or part number:_________________________________ Manufacturer's Name:____________________________________________ Manufacturer's Address:__________________________________________ (b) Offerors are responsible for submitting all additional information on the above product necessary for the Contracting Officer to determine whether the product offered meets the "brand name or equal" product's salient characteristics listed in the solicitation. (End of Clause) (xiv) All questions concerning this solicitation must be submitted by email to Lisa Draves at lisa.a.draves@aphis.usda.gov. Quoters are responsible for ensuring completeness of quotations. All pricing must be in whole penny amounts. The Government may evaluate solely on the information provided in the quotation. Quotations that reject the terms of the solicitation may be excluded from consideration. Alternate Pricing Schedules will not be considered. Quotations must include a price for all items in the solicitation to be eligible for award. The Government will award on an "All or None" basis.
