Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, 13 and as supplemented with additional information included in this notice. This solicitation is issued as a Request for Quotation (RFQ), solicitation number VA251-16-Q-0314, in accordance with FAR Parts 12.603 and 13.5. This announcement constitutes the only solicitation; QUOTES ARE BEING REQUESTED and a written solicitation will not be issued. Awards resulting from the (RFQ) will be based on Best Value to the Department of Veterans Affairs giving consideration to: Technical, delivery, Price, and Veteran's Involvement. This acquisition is Unrestricted, the North American Industry Classification System (NAICS) code is 334510, and has a size standard of 1,250 employees. The solicitation incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC) 2005-88, effective May 16, 2016. The Government intends to award Firm-Fixed Price. The requirement is for Brand Name or Equal. Delivery location is Cleveland VAMC, 10701 East Blvd, Cleveland, Ohio 44106 and shall be FOB destination. 1. BACKGROUND: The Louis Stokes Cleveland VA Medical Center is requesting three incubator, humidified infrared sensors. These CO2 incubators are used for incubating microbiology cultures Microorganisms recovered from culture must be "grown" in precise CO2 levels and at specified temperatures. 3.DELIVERY/PLACE OF PERFORMANCE: The equipment shall be delivered to the warehouse. The warehouse does have a delivery dock. The equipment shall be delivered within 30 days of the award date. Department of Veterans Affairs Louis Stokes Medical Center 10701 East Blvd. Cleveland, Ohio 44106 4. SPECIFICATIONS: "Exterior dimensions cannot exceed 80" H x 38" W x 33" L. "Large capacity ( 26 - 30 cu ft) of interior space "Temperature and CO2 alarms "CO2 range must encompass 5-10% CO2 "Temperature range: 5 °C to 50 °C "Minimum of 5 shelves 5. ADJUSTMENTS: All necessary adjustments (i.e., material defects, distortions, and back orders) shall be made by the Contractor at no charge to the Government and delivered within 2 days of notification by the VISN 10 Pathology. 6.BACK ORDERS: 8.a. Contractor shall notify the VA, within 24 hours after being notified by the manufacturer that supplies are on backorder. The Contractor shall provide an estimated date of delivery for backorders and shall notify the VA within 24 hours after backordered items are received. The preferred method of communication is electronic mail (e-mail). The Government requires the delivery of these items in accordance with the terms and conditions set forth in the contract. 8.b. Contractor shall substitute all backorders with a comparable equipment. 7.GOVERNMENT HOLIDAYS: The following are legal Government Holidays: New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. When one of the holidays falls on a Sunday, the following Monday shall be observed as a legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a legal holiday by the U.S. Government agencies. The provisions at 52.212-2, Evaluation-Commercial Items, add, Award Basis: The following factors shall be used to evaluate offers: technical, delivery, price, and Veteran's Involvement. Technical, delivery, and Veteran's Involvement combined is more important than price.
