- SAMPLE TASK ORDER RFQ CPSC-Q-16-0038 The U.S. Consumer Product Safety Commission (CPSC) intends to procure Peer Review, Toxicological and Risk Assessment Services of documents with a performance period of twelve (12) months for the base year and four (4) option years. This notice is a combined synopsis/solicitation for commercial services in accordance with Federal Acquisition Regulation (FAR) Subpart 12.6 and in conjunction with FAR Part 13.5, as supplemented with additional information included in this notice. This is a firm-fixed price type contract and is solicited as a Request for Quotation (RFQ) CPSC-Q-16-0038 for Peer Review, Toxicological and Risk Assessment Services as stated in the attached performance work statement. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The following provisions and clauses are enclosed in the solicitation attachment: 52.212-1, 52.212-3, 52.212-4, 52.212-5, 52.216-18, 52.216-19, 52.216-22, 52.217-8, 52.217-9, 52.227-14 and 52.232-18. All responsible sources may submit an offer which shall be considered by the agency. The Government anticipates that the requirement will fall under NAICS code 541690 with a small business size standard of $6.5 million. It is the responsibility of the interest ed parties to monitor FedBizzOpps for any amendments to this request for quotation. ALL QUOTES MUST BE VALID THROUGH October 31, 2016. The date and time for submission of quotes and any supporting documentation shall be received no later than June 23, 2016, 3:00 pm ET. All questions and/or clarifications in response to the solicitation shall be e-mailed to ggrayson@cpsc.gov no later than June 8, 2016 3:00 pm ET. No telephone calls please. If you are interested in this award, please complete the attached RFQ as follows: 1) Cover sheet (standard form 1449) - Block 17 with the contractor's name, DUNS number, address and telephone number must be complete. The signature, name and title of an individual authorized to commit the company must complete blocks 30a, b and c of the Standard Form 1449. 2) All offerors must have a Tax Identification number. System for Award Management (SAM) registration is mandatory to be eligible for this award. SAM is a self-service data base. Vendors register and maintain their own secure information through the SAM website.
