- Special Packaging Instructions AG00000573 Performance Specifications Technical Requirements Solicitation This Request for Quote (RFQ) is for the acquisition of a Fiber Optic Cable Assembly. This item shall be produced in accordance with the Technical Requirements Document, Exhibit A, and the Performance Specs, Attachment 1. 1. Offerors are to submit quotes for the following: NOUN: Fiber Optic Cable Assembly NSN: 6020-01-490-7024 P/N: In accordance with performance specifications A3302584 QTY: 136 each 2. The resultant Purchase Order will be issued FOB Destination, with Inspection and Acceptance at Origin for stock replenishment only. 3. This requirement will be a 100% Small Business set aside. 4. The ceiling for this requirement will be no greater than $150,000.00. 5. First Article Testing is required, unless a Request for Waiver for First Article Testing is approved by the Federal Government. See DLAD 52.209-9019, Requests for Waiver of First Article Testing Requirements (Sep 2008). Optical Cable Corporation (CAGE 6Y528), Federal Prison Industries (CAGE 64327), Fiber Systems International (CAGE 4DXK8) and Stran Technologies (CAGE 1HVU5) are waived from First Article Test Requirements. OFFERORS INTENDING TO REQUEST A WAIVER OF FIRST ARTICLE TESTING REQUIREMENTS SHALL SUBMIT ALL DOCUMENTATION SUBSTANTIATING THE WAIVER REQUEST WITH SUBMITTED PROPOSAL, AND MAY PROVIDE ALTERNATIVE OFFERED PRICES IN ACCORDANCE WITH PARAGRAPH (b) of DLAD 52.209-9019. 6. REQUIREMENTS FOR CONTRACTOR PERFORMED FIRST ARTICLE TESTING: As prescribed in section 4.3 of the Performance Specification and page 10 and 11 of the Technical Requirements Document, the contractor shall perform First Article Testing (FAT) in accordance with FAR 52.209-3. 7. The contractor shall provide a standard no-cost warranty. 8. This RFQ sets forth instructions to the offeror and evaluation factors for award. DLA will evaluate offers using FAR 15.101-2, Lowest Price Technically Acceptable Source Selection Process (See Instructions, Conditions, and Notices to Offerors or Respondents). 9. Accelerated delivery at no additional cost to the Government is accepted and encouraged. 10. Prospective offerors should insert a Firm-Fixed Price for the item number, complete any and all requirements within the RFQ, and sign and upload all pages (prior to the closing date) into the Federal Business Opportunities website.
