Description
The purpose of the amendment is to change the response date for quotes to Monday, 13 June at 10:00 am EST. This is due to the questions asked at the site visit. Questions and answers will be posted as an amendment to this solicitation. Submit offers via e-mail to janet.cubas@us.af.mil. This is a combined synopsis/solicitation for a commercial supply and/or service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. This is a Request for Quote (RFQ) and the solicitation number is FA4814-16-T-F012. This acquisition is a 100% small business set aside. The 6th Contracting Squadron, MacDill AFB, Florida, requires the following: DESCRIPTION OF REQUIREMENT: Location: 7561 Blackbird St. MacDill AFB, FL 33621-5101 SOCCENT Human Performance Building-1st floor building 1040 (non SIPR area) Overall Purpose of Project: This work is being requested to support EliteForm technology and well as PolarTeam technology which are major components of optimizing human performance and rehabilitation. Both technologies require internet capability to function. - 20 ethernet drops with associated power source(s) throughout room -- CAT5 or fiber optic - Cable will be run external to wall while being affixed to wall securely and covered with hard plastic sheathing. No cutting or drilling into the wall or ceiling. - 15 Surface Pro3 tablet holders will be affixed to wall securely utilizing industry standard techniques. Holders and surface Pro3 tablets already purchased by command and purchase of additional wall mounts is not required. - Facility will be assessed for need of switch, additional power needs and amount of materials (cord, etc) based upon SME recommendation and industry best practice upon site survey. - Any materials in excess which have been purchased by command will remain upon completion of service. - All cleanup and removal of debris will be completed by service provider. - Drops with be run to external server that will be separate from the existing SOCCENT network. A site visit will be held on Wednesday, May 25, 2016 at 9:00am. Upon receipt of this information, we will inform you of the location to meet us for the site visit. If you do not submit this information by the stated time, you are NOT eligible to attend the site visit. Only 2 representatives per company please.
